QF operated services within USA (eg LAX-JFK vv) not permitted unless booked as a through flight segment between Australia & USA (eg SYD-JFK vv)
My reading is that QF operated flights within the U.S. can be booked as a connection with a QF coded flight between Australia and the U.S. So, QF operated JFK-LAX connecting to AA operated LAX-SYD sold as QF would be OK, with a connection up to 24 hours.

Technically doable using long sell. I couldn't get availability display to display this "connection." However, from my reading it does not seem like this is allowed by the new rule.

On second thought, I think your reading is correct. The wording "a through flight segment" likely means only one segment.

Why is that not permitted? I can see QF not being permitted to sell JFK-LAX on its own, but why not in conjunction with a QF coded international flight?

From what the document says, because the flight LAX-PPT was operated by a different airline. Meaning on paper Qantas was selling JFK-LAX on its own (metal).

I just happened to pull up the fare rules in EF for a DONE5 ex-NRT on JL (likely these are identical for all xONEx, of course), and noticed a few things:

To my reading, this sounds like any ticket reissue, even for a change to carrier/date/time, costs $125. I realize that many airlines do a revalidation for such changes, but some (such as AA) reissue for everything. Also, in a case where D isn't immediately available on a flight but becomes available later, my understanding is that requires a reissue, as cabin changes can't be handled with a revalidation. It says that changes "are permitted provided ticketed points remain the same," but "are permitted" doesn't mean "for free."

Also, what does it mean that "if the rerouting results in a change to the total ticketed miles the ticket shall be recalculated?"

Thanks for the link, it explains the DOT rationale, which seems incorrect to my non-legal mind. I don't see why an international codeshare shouldn't count.

Wow, this is a biggie!I changed (reissued) my itinerary after the first leg 9 times during my last DONE4.I'd better plot it out more carefully next time before pulling the trigger.

Exactly, it is not clear. My guess is that the rules on the OneWorld site are what is intended, and the fare rules in the GDS are poorly written, but I wonder why the rules from the OneWorld web site were not copied as the GDS fare rules? I also wonder if a travel agent or airline might use the GDS rules and might charge $125 for each reissue?

Cabin is not a concept that appears on a ticket. A booking code may be changed by a revalidation. See Resolution 727.

It does unless the rules say otherwise.
