i found it on the diplomatie website that was briefly online with the form to send. It is not the same form as french nationals.

Any chance you have a copy of the form you could post (without your personal data of course)

See Demande de passe sanitaire en cas de vaccination à l’étranger (procédure pour les touristes de passage en France, hors UE) - Ministère de l’Europe et des Affaires étrangères (diplomatie.gouv.fr) and the linked form. It's in French, but seems aimed at tourists.

Thanks!

For those who are bad at French (i.e. me) what goes in the "A _________, le ___________" line at the bottom of the PDF?

This is one of the FAQs on the website:

Will the health pass thus obtained be recognized in other European countries?

Yes. It is generated in a DCC digital format recognized throughout the EU and countries that have established a mutual recognition agreement with the EU. Find here the list of countries concerned.
That means getting the QR code is basically the equivalent of a EU Digital Covid certificate?

Correct. Import into the French app or any other compatible European app.

These are the emails
To facilitate the processing of requests received, the email must be sent with a title worded as indicated below:

The English language version of the instruction page and form is now live: Applying for a COVID certificate if you were vaccinated abroad (procedure for non-EU tourists) - Ministry for Europe and Foreign Affairs (diplomatie.gouv.fr)

The earlier site indicated that the tourist pass, like the one for French nationals, would not have an expiry date. So, in particular it could be used for multiple trips. I cannot find any mention of expiry on the latest site. Does anybody have any knowledge of that?

It would be great to hear from somebody who actually gets a pass -- whenever that might be!

It looks like they just opened it up to tourists. They talk about using the form for french nationals but I cannot find the form anywhere on the embassy websites or the diplomatie site. [emphasis added]
Can you tell me where you found the form?
Thank you

Just FYI, this form is the French National form. Might still work I guess but as you can see it doesn't mention the plane ticket, requires French passport and proof of residence (which you don't need for new process)

I gave Diego37921 exactly what he asked for and at that time, several hours ago, this was the only form available.

Messed up the subject line

Hi All, I did not see the English instructions until after I submitted the pass. As a result I did not see that there is a specified format for the subject line including the name, date of arrival etc. Yet, I did receive the response that my email was received and I should not submit it again. I did attach all of the required info.

So will the faulty subject line mess up my application? Has anyone successfully submitted the email without the specific subject line format and received a QR code? I don't want to receive duplicate codes that end up being invalidated. Thanks!

That was easy. Sent and received an immediate acknowledgement. I don't arrive until September 8th so it will be interesting to see how long it takes.
