My understanding was that the aisle/window option only was available at OLCI if you have automatic C+ upgrades deselected in your profile.

Unless you are thinking the GA called them up to inform then they would be in a middle C+ seat and they declined. Or maybe all 3 went to the GA and declined the middle seat upgrade (never mind the other two seats were 10 and 11 on the aisle).
Odds of this happening are less than me getting an upgrade on an ATL-PBI flight IMO.
Agent still ask as unless passenger goes and manages the selection online they'll be marked as wanting to upgrade.

I check that box and have appeared on the list anyway, but have never gotten a middle seat.

This is consistent with my experience. I do SDC a lot so I end up in regular main cabin pretty frequently at checkin. I have automatic C+ upgrades deselected, and I always get asked if I want a middle at OLCI.

Might anyone be able to tell me how two standby passengers could receive Delta One upgrades in front of two passengers on the upgrade list?

On the flight from LAX-JFK, all settled so that there were 3 Delta One seats available for upgrade. To give you a sense of who might be at the top of the list, I wasn't top 3 even though I'm diamond and was applying a RUC.

The standbys couldn't be SDC, as SDC would automatically be assigned the seat vs. being put on standby.

Not if the only seats available are blocked, as has happened to me.

I will grant you that, I don't know how common that situation is though, so I'm not sure if this would often be the legitimate explanation, or if it is still more likely to be Shena.

This happened to me a few weeks ago; DAL said it a confirmed D1 pax who just hadn't selected a seat when purchasing the ticket.
