Looking at the one way economy fare rules there is at least a $15 penalty on all fares

Yes it is. Doesn't matter if its a flex fare in J or F or Y, most EK fares have cancellation charges. I have only seen a few F class fares which didn't have a fee.

Conversely I have seen few fully flexible fares which do have a cancellation charge

Regardless, purchasing a ticket, checking the fare rules before purchase is the sensible approach

I will repeat this again: IT'S NOT IN THE FARE RULES!

It's not in the fare rules I pull up on ITA, EF, EK, or the rules they sent with the e-tickets. Like I said before, the only mention of a cancel fee is for no-shows. This is not a issue of the fare not allowing a full refund with no charge. My question is, does EK have a blanket refund fee even for refundable tickets?

khalid26 says they do, but I cannot find that anywhere on the EK website.

Don't jump on me, my comment was a general comment with the difference between my rarely finding penalties vs his generally finding them

I have looked at the current rules and I also cannot see any $30 fee for business class tickets; I saw $15 mentioned on all economy fares , but nothing regarding a $30 on business fares

I cannot see anything that would limit the refund unless the wording is the weird bad grammer that fare rules can have

I would understand that as being LKR4000 for a no show ( approx $35 )

CHARGE LKR 4000 FOR NO-SHOW.

but is that sposed to be read as "CHARGE LKR 4000" and then goes on to start the NO SHOW part

If it is the latter , then would be $35

There is no general refund fee; I have cancelled and claimed refunds on tickets and received 100% refund where there is no cancellation penalty; I have been affected by exchange rate changes and credit card foreign currency fees where it has been in another currency, but as far as a penalty from EK, nothing that wasnt listed in the rules

No that is not what I said. I said it is standard for EK to have cancellation/refund charges on their tickets even if they are flexible and/or in premium cabins so you shouldn't be surprised.

Now if your fare conditions state no charge and EF confirms it then you should reply to the Male office email attaching your fare conditions and asking for your $60 back.

Can you share the fare basis code so we can verify this?

Sorry, this whole process is frustrating me. I don't like trying to do this over email. The phone agents cannot help. The EK Maldives office said I owed the fee per some unspecified document. I have emailed back asking for clarification and haven't received it.

I did email the EK Maldives. Here is their reply:
Dear skunker,

I don't know what this "document" is. I replied asking for clarification and attached the fare rules that EK sent me which state no cancel fee. The fare code is IOWMV1 and has similar rules as the one Dave Noble posted. I believe his is the CMB-MLE one-way vs MLE-CMB.

I can see that in surcharges there is a $25 fuel surcharge; perhaps that is not being refunded

I disagree here; some premium fares have cancellation penalties and some don't; there is no way that I would say that it is standard for there to be cancellation penalties

Ridiculous wait

I canceled an F ticket (the fare rules said refundable, and I didn't notice a cancellation fee -- we shall see), then filled out the on-line refund forum. Nine days later, I got an e-mail acknowledging the submission and saying that they would process the refund -- with the funds returning to the card in another 21 days (plus any extra time that the bank takes).

It sounds from others' experiences here that this is a conservative estimate, but, still, this is completely outrageous. It should be instantaneous -- it certainly is when they take your money.
