Quoting a chain for a menu item is blasphemy in my book but Panera does a tolerable version of the Cuban sandwich. (I have a client who LOVES Panera & when we go to for lunch this is my sandwich of choice)

San Daniele sandwich at "Salum" - New York.
An amazing sandwich (or panini as you call it in the US): ciabatta bread, truffle oil, san daniele ham sliced fresh to order, parmigiano reggiano slithers and mayonnaise.
Salum is really worth a visit if you're in NY or you pass by.. All their sandwiches are amazing.

I wish Costco had healtier selection of choices.. Their apple pecan salad with chicken breast is quite tasty.. but the other meal selections are all high in cholesterol and fat..

Luckily, there are too many to count. But I'll contribute a few:
I wish I could remember the name of the caf, but in Pisa, Italy, I had a great roasted aubergine (eggplant) and buffalo mozzarella panini, alongside a San Pellegrino Aranciata. I'd like to give an extra off-topic thanks to the readily available mascarpone and nectarines from that time...
The mujaddara sandwich from Kalustyan's in Manhattan, New York. Lentils, onions and groats, with a side of pickles - well done indeed.
Broccoli rabe, provolone and roast pork at Tommy DiNic's in Philadelphia. Ate it a few days ago, should've been sooner than that.
Many varieties of Yamazaki pan (bread) in Japan. Peanut butter, maple syrup and margarine (uhh), katsu, to name but three. Never refrigerated, keeping it classy.
Pretty much any sandwich I've come across in the Levant is worthy of this reply.

I have a new contender I made last weekend:
Two slices of multi-grain bread
Roast pork loin
Mashed potatoes
Oh it was amazing!
