Pho in Kansas!
There were several really good Pho shops in Wichita KS:
Pho 99 on Harry is good... And just outside the best Thai-Asian market in the state (Bihn Thai on 21st street) there's a little no-name shop that dishes up great Pho and Tom Yum Kun.

Good Pho place in San Diego is on Claremont Mesa blvd near the Tommy's burger joint. I think the name is Ho Cali... it's a very large joint and also very local.
Another good place in Mira Mesa is Pho Ca Dao. It's on Mira Mesa blvd near MM HS just beyond Camino Ruiz if going east bound.
I want to find some good Pho in Delaware/Philly.

I'm disappointed in the lack of pho places in downtown Denver, but An's Lemongrass Grill serves some great pho. The portion is pretty small, especially price-wise compared to most of the others, but the broth was very flavorful.
In south Denver, Rolls & Bowls in Lone Tree is decent.

Le Saigon - W. Los Angeles
I used to work within walking distance of this place and eat here at least once a weeks. I've seen some mixed reviews of it on the internet, but I have only experienced great meals there.

Just IMO of course:
My typical order: Pho Chin Nam Gan (cooked brisket, cooked flank, and tendon), with a side of hot chili oil for dipping in the hoisin sauce.
-Pho Hoa (US) is crap.... way too much fatty broth.
-Pho 24 (VN) also crap. The broth tastes weird, as does the hoisin sauce.
-Favorite mom-pop pho store is Golden Deli in San Gabriel, CA.
-Favorite mom-pop pho ga (chicken pho) is a place by the Stater Brothers on Brookhurst and Hazard in Westminster, CA.
-My favorite chain is Pho 79... try ordering a bowl of their soup bones.
-Best pho in Saigon is Pho Hoa Pastuer, on Pastuer (but do NOT eat the condiments on the table... both times I have, I've had "digestive issues," but without the condiments (I'm guessing the hoisin sauce) I've been ok. Bring your own if you want.
-In general, the best pho is in California... nowhere else in the US has it been good.
**My mom's pho trumps all of the above

There are many pho restaurants all around the Silicon Valley, particularly on El Camino Real, east and west. In San Jose, you find pho being offered in all neighborhoods. I like the well-done beef with soup noodle. Most places are jammed with office workers, during lunch hour on week-days. Its cheap, fast and good!

There's also a thread I started on this topic about Pho in Paris, another enclave of fantastic Vietnamese food:

Of course, UA flies one-hop direct to SGN now.
You can get the real stuff right off a real carrier.

I've been to at least 20 pho places in the South Bay, and two really stand out:
Pho Kim Long, on Capitol just south of the great mall near Milpitas, has it all. Their noodles are the best, the other ingredients are great, the bowls are huge even by Pho standards, the prices are low even by Pho standards, it's always packed by a mostly Asian crowd, and the name is really funny (find out the correct pronounciation of "Pho").
Near the SJC airport, there's a great place on 1st just south of 880 called Pho Bac So, which also has a bakery counter called Chez Croissant. You can check in for your flight, head over for lunch (or breakfast -- they open at 8), grab some pastries for the flight, and head back to the airport. Their broth is strongly flavored with warm spices; I've been told this is a more northern style. I've taken some Skywest pilots there and they eat there every time they're in town now.
There's a surprisingly good place in Reno, just south of Cal-Neva, called Pho 777.

Hey, is there any validity to the urban legend that the number in the restaurant's name is the year the owners immigrated from Vietnam?
