Some banks (if you already have a relationship with them) let you apply via mail. Sometimes you have to be a resident of the state they operate in, while some were able to get the card without living in those states. It is very much YMMV. You will need proof of income (W2, check stubs, etc). The only states City National Bank operates in are: NY, DE (only trust office, no go), DC, GA, TN, MN, NV & CA. The bank is looking for high net worth individuals, but that's not exactly consistent either.
The card has no foreign transactions fees and is fee-free the first year, then $400 annually.

I didn't have my AU with me in the branch. I gave her name in the branch when I applied for the card, but then I had to follow up by e-mailing my relationship manager a scan of her license and her mother's maiden name.
You can probably do it after you get the card but you will most likely still go through your contact at the bank. I've never had experience calling them.

Separate numbers, cards arrived same day in separate FedEx overnight envelopes. I can't answer your other question directly, but the rep didn't seem to care if my AU was related to me.

probably different in my case because the airfare itself was less than $100 so they can not put the $100 deduction from one ticket...

It's in the same tab as Transactions. On the top right corner, there is section says Review Authorizations. You can see all approved and declined transactions after you click the Go button. It doesn't say pending, but if it's approved and not showing in transaction list yet, then it's basically pending.

I am new to this CNB Crystal Visa card so please forgive me newbie question.
Looks like we can book an international WN flight for its high tax/fees. CNB will refund the card as rebate. After the rebate we will cancel the WN flight.

DP for those still trying to use their airline credits-- PGGM for JetBlue seat upgrade fees (refundable fares) works as well
