Sometimes you have to be a resident of the state they operate in, while some were able to get the card without living in those states. It is very much YMMV. You will need proof of income (W2, check stubs, etc). The only states City National Bank operates in are: NY, DE (only trust office, no go), DC, GA, TN, MN, NV & CA. The bank is looking for high net worth individuals, but that's not exactly consistent either.
The card has no foreign transactions fees and is fee-free the first year, then $400 annually.

Looks like some of you are coming back to this thread after doing PGGM earlier in 2016. I can tell you whats working and whats not.
PGGM stopped working in mid to late 2016. Trust me, I tried it after adding some Authorized Users.
AA gift card (electronic or Physical) still works.
WN award purchase + Tax (as long as its around $30-$70) works.
Thats what I have been doing. I have two accounts with 8 authorized users total. I Just paid $800 annual fee today and hoping to get $2000 in Fees back by cancelling southwest Award booking flights over and over again.

The last one I did for 2016 with WN PGGM was 12/30/16 and I see the rebate on 1/6/17 although the date for the rebate was 1/5/17. Maybe if I had checked late on 1/5, I would have seen it? not sure. You are likely to see it late tonight, if not, tomorrow morning. Good luck!

It usually takes longer if you do it over the weekend, and 1/1 is holiday, 1/2 could be as well, so it would be longer.
