Just got this reply:
I apologize for the delay in responding to your email.
United recently changed procedures for provisioning pillows and blankets
on board our aircraft. After several weeks of study and evaluation of
employee and customer feedback, a new provisioning procedure has been
developed that place clean and wrapped blankets re-cased pillows on our
aircraft.
As with any business, we consider customer preferences and monitor
customer feedback closely. Your comments about our pillows will be
shared with our management team and will help us evaluate our decisions
that impact your choice of airlines.
At the conclusion of this email, please fill out the survey where you
can rate your experience with the Customer Care department. We value
your feedback and hope that you are satisfied with our service.

and more info, looks like we are making progress!
Below is the new information I have for the provisioning of pillows and
blankets.
Transatlantic, Transpacific, South America and Transcon (ps) flights:
Pillows and blankets will be provisioned for every customer traveling in
First, Business and Economy.
To/From Hawaii, overnight Transcontinental and North America flights
operated by 3-cabin aircraft: Pillows and blankets will be provisioned
for every customer traveling in First, Business and for most customers
in Economy.
Within North America (exception above): Pillows and blankets will be
provisioned for every customer in First and Business only. At this time
there is no provisioning for the Economy cabin.

Had pillows and blankets in Y both ways on SFO-ORD on the 3-class 767. Maybe holdovers from the trans Atlantic legs?

SAN-ORD 7/27. No pillows on the red eye. Two types of blankets though. One was a crib blanket sized, gauze blanket. The other was double the size, but felt like a polyester table cloth.
