The requirements for the application process varies from bank to bank based on various online data points collected from FlyerTalk, Doctor of Credit, etc.
Some banks require that you go into a branch and apply personally. Other banks want you to have another account open for 90 days before you can get this card. Some banks (if you already have a relationship with them) let you apply via mail. Sometimes you have to be a resident of the state they operate in, while some were able to get the card without living in those states. It is very much YMMV. You will need proof of income (W2, check stubs, etc). The only states City National Bank operates in are: NY, DE (only trust office, no go), DC, GA, TN, MN, NV & CA. The bank is looking for high net worth individuals, but that's not exactly consistent either.
The card has no foreign transactions fees and is fee-free the first year, then $400 annually.
Register your card for online access (cno.cnb.com). This changed on Sept 19 2019.

i had the same issue with the gogo passes, and they showed up about a month later with no action on my part. not sure if CNB intervened, or if it just takes a while.
unfortunately, i'm not getting any credits from AU airline charges. i've had $250 worth of credits applied to several $50 united GR contributions on my card, but none from my wife's card.

Datapoint for others. Used PGGM bought flight (and seats)
Transaction was on the 19th, posted on the 21st
12/18/2015 12/21/2015 DELTA AIR SEAT FEES $15.00
12/18/2015 12/21/2015 DELTA AIR SEAT FEES $25.00

12/18/2015 12/21/2015 DELTA AIR SEAT FEES $139.00
I then CNX the flight about 4 hours later and this posted like this:
12/18/2015 12/21/2015 DELTA AIR SEAT FEES $15.00
12/18/2015 12/21/2015 DELTA AIR SEAT FEES $25.00

12/18/2015 12/21/2015 DELTA AIR SEAT FEES $139.00
Now today when I login, got the rebate posted (up to the $250 max per card)
Note I had logged in last night at around 2200 Pacific and this was not there.
12/19/2015 12/23/2015 TRAVEL FEE REBATE $8.00

12/19/2015 12/23/2015 TRAVEL FEE REBATE $139.00
As can see, at least one was above $75, so seems can exceed that. We just have to wait a few days before you get the rebate.
Also on 21 Dec, got my min spend bonus, got a 1k gift card, ordered on the 21, processed on the 22, got it via FedEx today (24th) was shocked to get that quick, considering the holidays. Clearly the fulfillment department or company is quick with getting the GC out.

I came here to ask the same question.
I also ran the PGGM on 18 Dec. Seems to always take a day or two to show up (seems this card system does not have a "Pending" where you can see it) So about 4 hours after I purchased the tickets (and seats) I Canceled the flight. Then on the 21st see the same as you (except total for my seat for the legs was 257.00. Don't see any $250 credit. Just wondering how many days to wait or if this may not work after all? And need to find another avenue.

More data point on Southwest!
Got Travel Rebate for $61.74 for Southwest Award fees, but no credit for award fees charge of $90.74. I have made another award fee charge of $67.34, so we will see if the credit comes through for this one. Meanwhile, best it seems to do Delta PGGM as it seems to work even for a charge of $139.

So far I've gotten credits for PGGM for Delta and SWA on my primary card. I did PGGM for AU card using SWA and haven't gotten credits. Using primary card, I did SWA transaction on 12/14 and was credited 12/23. Using AU card, I did SWA on 12/15 (1 day later), and still nothing.
Those of you who got the airline credit on primary as well as AU card, did it take longer to get the AU credit than the primary card?

They do show pending transactions. Select "Review Authorizations", then select number of days frompull-down menu.
