Read me first!
All airlines (although see note about TG) appear to give them departing from Bangkok, so the list below is mostly useful for inbound. It's important to remember that this is Thailand so just because an airline has given them out before is no guide they'll be given out in your case. It's worth asking both air-crew and the ground crew at the gate for them, as they're also occasionally forgotten. Thus the list below tends to reflect "this airline has given them out at least once" rather than being any guide to what will actually happen...

AI (Air India) - J arrival and departure
AF (Air France) - J arrival (distributed in the jetway leaving the aircraft), J and W departures
AY (Finnair) - J arrival (jetway) and departure
BA (British Airways) - J arrival and departure (BE AWARE there have been instances where cabin crew have not handed out the pass and when asked either only given to the asking passenger or advised they aren't given out)
CX (Cathay Pacific) - J/F arrival and departure, OWE departure
EK (Emirates) - J/F arrival and departure, Gold and above in Y excluded by local rules
EY (Etihad) - arrival (criteria unknown)
HU (Hainan Airlines) - J departure only
HX (Hong Kong Airlines) - J departure and J arrival
JL (Japan Airlines) - J/F arrival and departure
KE (Korean Air) - J arrival and departure, assume F as well
LX (Swiss Airlines) - J departure
LY (El Al) - arrival and departure for J+F plus KD
MH (Malaysian Airlines) - J arrival and departure, OWE departure

OZ (Asiana) - J arrival and departure (no OZ F service in BKK) (no arrival 26/03/16)
PG (Bangkok Airways) - Blue Ribbon (C) or Premier ongoing for arrival and departure
QR (Qatar Airways) - J arrival and departure, OWS (or above) departure
RJ (Royal Jordanian) - J and OWR (or above) departure, J arrival,OWR (or above) in Y (if you manage to find RJ staff at the jet bridge)
SQ (Singapore Airlines) - J arrival and departure or PPS Club/Solitaire, sometimes on request only
SU (Aeroflot) - J arrival and departure
TG (Thai Airways) - F/J have dedicated security area at airport for outbound, for arrivals not given but your boarding pass is sufficient
TK (Turkish Airlines) - J arrival
UL (Sri Lankan Airlines) - J/OWS (or above) arrival and departure
VN (Vietnam Airlines) - J departure only
Notes:
1) Immigration seems to only care about the airline stamp. Recent experience continues to be hassle free when the printed date and/or names do not match or are not filled in. YMMV. Edit: In early June (2015) was told by dragon to fill in the name field on the Fast Track voucher. She then checked to see if it matched BP name.
2) If you're inbound to BKK and have not received a voucher onboard the aircraft, be sure to ask!
3) Fast track vouchers do not have to be used on the date issued. If regular immigration queues are short on your arrival date, it's often worthwhile to pocket the voucher for a different arrival date when you may not have premium access (or to give as a gift to friends/family coming to visit).

Sorry - not an answer to your specific inbound question but NH does give out Fast Track passes outbound. They have a stack of them at Business check-in desks. It may then be logical that (maybe not as well) they give them out inbound.

Generally I think any airline that provides them on the outbond should provide them on the inbound,some of them probably don't advertise it and you don't get it unless you ask,especially if you are elite in Y.So asking the staff on arrival is a good strategy. Note that I say they should,which is often different from whether they actually do it.

I don't remember ever getting one from UA when arriving in Y (as 1K/*G). Same on NH when in Y.
On CX I've been successful in asking the purser during his/her meet and greet for OWE pax but its not a written policy so far as I'm aware.

I mean,that's a very cheap sort of behavior,the voucher costs the airlines 50THB.

Did you mean 'zone 2' upon entry. I mistakenly joined the Chinese line on the left side earlier ago. Two Chinese pax threw in some extremely offensive remarks at me (about being Chinese and holding a foreign passport(/in the wrong line maybe?)...duh) but I just ignored them, whatever. At least the immigration officer didn't turn me back and it just took a bit longer time than the right side lines.

I think them being so close is unhelpful; there's a lot of bleed from the Chinese queues in to the Premium queues. Also an unusual number of economy pax not apparently holding vouchers yesterday, although I only had the documents in their hand to go on, so who knows?

Fast track pass is always in the pocket out of site. I don't want to hand it over, and figure this might help to reduce the chance of beng asked for it.

It's one thing if they don't have them at alll...not for J,nor for the cardholders,but if they have some for J,I would be surprised if they refused a top tier cardholder a voucher.

Then your experience is diametrically opposed to mine.
With a stash of ~60 passes currently I'm not concerned nor can be bothered chasing down ground staff on the rare occasions I don't get one on board. Chances are the scheme will change before I have the opportunity to diminish my stack anyway.

Since J naturally J disembarks before the others,they just hand them the vouchers,but I never had a problem getting one by presenting a Y+Emerald BP.
Other airlines hand it onboard (like CX),yet according to a few trip reports there are some airlines who even do it at the check in counter in their base airports (eg:MK),which I find very unreliable,since not everyone is visiting the check in counter.
PS:I am trying to build this kind of stock (it became sort of a stupid hobby when flying to/from BKK) and maybe once I collect 60 vouchers I will stop being caring about it too
