All airlines (although see note about TG) appear to give them departing from Bangkok, so the list below is mostly useful for inbound. It's important to remember that this is Thailand so just because an airline has given them out before is no guide they'll be given out in your case. It's worth asking both air-crew and the ground crew at the gate for them, as they're also occasionally forgotten. Thus the list below tends to reflect "this airline has given them out at least once" rather than being any guide to what will actually happen...

List

AI (Air India) - J arrival and departure
AF (Air France) - J arrival (distributed in the jetway leaving the aircraft), J and W departures
AY (Finnair) - J arrival (jetway) and departure
BA (British Airways) - J arrival and departure (BE AWARE there have been instances where cabin crew have not handed out the pass and when asked either only given to the asking passenger or advised they aren't given out)
CX (Cathay Pacific) - J/F arrival and departure, OWE departure
EK (Emirates) - J/F arrival and departure, Gold and above in Y excluded by local rules
EY (Etihad) - arrival (criteria unknown)
HU (Hainan Airlines) - J departure only
HX (Hong Kong Airlines) - J departure and J arrival
JL (Japan Airlines) - J/F arrival and departure
KE (Korean Air) - J arrival and departure, assume F as well
LX (Swiss Airlines) - J departure
LY (El Al) - arrival and departure for J+F plus KD
MH (Malaysian Airlines) - J arrival and departure, OWE departure
NH (ANA) - departure, J also arrival (4/2016)
OS (Austrian) - J departure
OZ (Asiana) - J arrival and departure (no OZ F service in BKK) (no arrival 26/03/16)
PG (Bangkok Airways) - Blue Ribbon (C) or Premier ongoing for arrival and departure
QR (Qatar Airways) - J arrival and departure, OWS (or above) departure
RJ (Royal Jordanian) - J and OWR (or above) departure, J arrival,OWR (or above) in Y (if you manage to find RJ staff at the jet bridge)
SQ (Singapore Airlines) - J arrival and departure or PPS Club/Solitaire, sometimes on request only
SU (Aeroflot) - J arrival and departure
TG (Thai Airways) - F/J have dedicated security area at airport for outbound, for arrivals not given but your boarding pass is sufficient
TK (Turkish Airlines) - J arrival
UL (Sri Lankan Airlines) - J/OWS (or above) arrival and departure
VN (Vietnam Airlines) - J departure only

Notes:

1) Immigration seems to only care about the airline stamp. Recent experience continues to be hassle free when the printed date and/or names do not match or are not filled in. YMMV. Edit: In early June (2015) was told by dragon to fill in the name field on the Fast Track voucher. She then checked to see if it matched BP name.

2) If you're inbound to BKK and have not received a voucher onboard the aircraft, be sure to ask!

3) Fast track vouchers do not have to be used on the date issued. If regular immigration queues are short on your arrival date, it's often worthwhile to pocket the voucher for a different arrival date when you may not have premium access (or to give as a gift to friends/family coming to visit).

I've recently been (mostly) successful in getting an FT card when flying Y/Y+ on CX into BKK.

As an AA ExPlat (OWE) I have simply inquired when the FA/Purser introduces themselves, gives me a bottle of water and asks if there's anything they can get me.

KE gave me one **inbound** today-- a first in all my years of flying KE, which does reliably give out the cards outbound.

I tried to use one of my 'saved' FT passes the other day arriving from SGN on VN, which didn't give me one on the inbound. The immigration official went ballistic on me; asked me in a disgusted voice where I got it, how I got it, why was I trying to use a pass from one airline when I arrived on another. Seemed like she wasn't going to admit me so I finally smiled and said, "Screw it. I'll just use the regular line...no worries. May I have my passport back, please?" She shook her head, looked even more disgusted, and admitted me. When she handed my passport back, she waved the pass at me and said something that I didn't understand and totally ignored.

I probably won't try it again, although I have friends who do it on a regular basis and never have a problem. In the end, she was a right to question it, I was wrong to try and use it and I got busted. End of story.

It does say "This coupon is valid only on flight/date shown", and "Not for Sale", but AoT did sell these, and get the revenue for them from the airlines, CC partners, banks, etc., so it'd be nice if you could "reserve" them.

Out-bound the airlines seem to fill them out in great detail (name, flight, date, etc.) while in-bound they are sometimes blank, or just have a 2-letter airline code stamp.

You can buy blanks on-line here (400 THB plus 100 THB delivery fee). Many airport/airline employees have stashes of these, and I was given ~ 10 blanks.

I've used reserved passes in the past without the IO flipping out, but if I were challenged I might say that I am sorry, I simply pulled out an older one by mistake, and then I would present a blank, valid pass. Whether this would work I cannot say, as they might push back re: where did you get this pass, and compare your class of service on your BP (which they can request)?

The RTP/Immigration front-line postings at SBIA/BKK are not particularly desirable, so it's easy to understand why some might get upset.

Heavily airline dependent. Last JAL flight I took it was perfectly filled out, yet to have QR one that had anything more than a stamp. Last few journeys I've had the person checking them at security ask me to fill some details out on them about 50% of the time.

PG sometimes fills them out and sometimes doesn't.

Other than one time I was asked to write my name in there's been no further requests that I've received and I've also noticed other pax handing over completely empty cards

For arrivals it's rare I use a card that has any info filled in save the airline stamp.

Philippine Airlines (PR) has given the passes to me as PR Premier Elite and my guest with no status, departing in economy class. No such luck on arrival.

Has anyone accessed this lane with BP showing a Fast Track label, traveling in a Premium cabin, or showing a plastic airline elite card wihout the "golden ticket"?

I haven't but I know a couple of people who tried. Immigration is not interested in your elite status. They are interested in whether your airline paid for the privilege. No card, no entry.

I think the poster you quoted was putting forward 3 scenarios so your answer doesn't really make sense except to the third one.

I would have thought:

1) A Boarding Pass doesn't show a Fast Track label.
2) Travelling in a Premium Cabin should get you a pass if your airline has negotiated it (see the Wiki).
3) Elite card means nothing.

ETA. Airline elite status card, not Thailand Elite card.

In addition to the airlines, local promotions and credit card affiliated "promotions", I believe the Thailand Elite card/program continues to offer Fast Track services, regardless of airline/CoS.

And I think some local credit cards, on the high end, offer this feature, outside of any promotional aspects. But I haven't reviewed those in a while.

Also, include the APEC Business Travel Card (ABTC) on this list. Inbound/outbound fast track access at both DMK/BKK.

Having always flown into BKK on business or first, I just use the fast track lane even if not given a stamp. No one gives me trouble, but sometimes when they ask I just say I'm in premium cabin and show my boarding pass. Although when I fly in on TG F, I'm always escorted to the fast track by an airline employee.
