Read me first!
All airlines (although see note about TG) appear to give them departing from Bangkok, so the list below is mostly useful for inbound. It's important to remember that this is Thailand so just because an airline has given them out before is no guide they'll be given out in your case. It's worth asking both air-crew and the ground crew at the gate for them, as they're also occasionally forgotten. Thus the list below tends to reflect "this airline has given them out at least once" rather than being any guide to what will actually happen...

AI (Air India) - J arrival and departure
AF (Air France) - J arrival (distributed in the jetway leaving the aircraft), J and W departures
AY (Finnair) - J arrival (jetway) and departure
BA (British Airways) - J arrival and departure (BE AWARE there have been instances where cabin crew have not handed out the pass and when asked either only given to the asking passenger or advised they aren't given out)
CX (Cathay Pacific) - J/F arrival and departure, OWE departure
EK (Emirates) - J/F arrival and departure, Gold and above in Y excluded by local rules
EY (Etihad) - arrival (criteria unknown)
HU (Hainan Airlines) - J departure only
HX (Hong Kong Airlines) - J departure and J arrival
JL (Japan Airlines) - J/F arrival and departure
KE (Korean Air) - J arrival and departure, assume F as well

LY (El Al) - arrival and departure for J+F plus KD
MH (Malaysian Airlines) - J arrival and departure, OWE departure

PG (Bangkok Airways) - Blue Ribbon (C) or Premier ongoing for arrival and departure

RJ (Royal Jordanian) - J and OWR (or above) departure, J arrival,OWR (or above) in Y (if you manage to find RJ staff at the jet bridge)
SQ (Singapore Airlines) - J arrival and departure or PPS Club/Solitaire, sometimes on request only
SU (Aeroflot) - J arrival and departure
TG (Thai Airways) - F/J have dedicated security area at airport for outbound, for arrivals not given but your boarding pass is sufficient
TK (Turkish Airlines) - J arrival
UL (Sri Lankan Airlines) - J/OWS (or above) arrival and departure
VN (Vietnam Airlines) - J departure only
Notes:
1) Immigration seems to only care about the airline stamp. Recent experience continues to be hassle free when the printed date and/or names do not match or are not filled in. YMMV. Edit: In early June (2015) was told by dragon to fill in the name field on the Fast Track voucher. She then checked to see if it matched BP name.
2) If you're inbound to BKK and have not received a voucher onboard the aircraft, be sure to ask!
3) Fast track vouchers do not have to be used on the date issued. If regular immigration queues are short on your arrival date, it's often worthwhile to pocket the voucher for a different arrival date when you may not have premium access (or to give as a gift to friends/family coming to visit).

The dragons guarding the departure access will check the DOB in the passport. There are usually no dragons on the Arrivals queue

Word in other forums on FT is that AoT has scrapped the passes altogether and now requires an F or J BP to access the Fast Track channels. Is that right?

Correct, Mega. Means that elites traveling in Y no longer have access. Was outbound the other day, and security/immigration were both delightfully empty.

I first used it in 2010 so it only took them 9 years to make a J/F Fast Track available to only J/F. Pretty snappy performance.

Huh? That wasn't the original goal. Originally it was intended for elites as well as premium pax, as in many other airports. It's not as if they intended to do this in 2006 when the airport opened and just got around to it now, 13 years later.

I arrived on Philippine Airlines today and they said that Fast Track Vouchers were no longer available, and that therefore we could not use Fast Track. I do not know whether it was just a miscommunication or what
In the end did not matter for me as there was no queue at the regular immigration channels

To back up these recent comments, last week I went to Hong Kong on Cathay Pacific in business. At check in at BKK I asked for the blue card, was told wasn't required and guards at fast track checked our boarding cards and noted we were in business and let us through. On the way back, no cards given out on the plane. We headed to the fast track immigration section and I said to the guard there that we don't have blue cards, he said not required, checked boarding passes for business and let us through.

Went through BKK AP last week and as an Elite passenger in Y I was refused a card both entering and leaving. It was explained that AOT was enforcing the requirement that only J/F pax were allowed to access the express queue.
Fortunately the lines were quick both times but I would hate to be there when it was frantically busy.

Will be flying into/out of BKK from/to AMS via XMN on a XiamenAir (MF) Business Class ticket.
Does MF give Fast Track passes to their J passengers? Hope to avoid the crazy long foreigner passport control queues on BKK arrival.

There are no more fast track passes. Fast Track access is determined by cabin. Premium cabin boarding passes are all you need for fast track access. Elite status is no longer relevant.

Anyone got any experience of using non Thai Airways Fast Track with Thai Airways Business Class boarding pass? I would like to use it as I can use the autochannel and from this Fast Track there is a door leading through to the autochannel.

Can Fast Track for immigration for DEPARTURES be purchased? I have a morning flight (on TG econ, international) at 8am. I heard that there are lots of long haul flights departing in the morning, and therefore immigration and security lines are crazy.
