Acrobat Reader has a feature "Fill & Sign". Not sure where it shows up in different version, but for me it is just in the main menu under "Sign". It works for forms even which are not set up of with those fillable text boxes.

Yes for me it took a full week (while others receive more quickly; then again others aren’t receiving at all so at this point I’m just grateful).

Just submitted my paperwork to get the Pass Sanitaire.
I thought the process was pretty smooth. Now let's see how long to get the QR code.

edit: received the conversion PDF file 12h after submitting.

(dual citizen French-US!)

I re-submitted my application and got a response within 5 min! This was approx. 2pm in Paris.

Did you change anything?

Change the subject line? The attachments?


Some EU countries are starting to put an expiration date after the second vaccine dose on whether people can enter. Croatia and Austria so far.

Inevitable other countries will also impose expirations.

What happens with the pass sanitaire, when it comes time to enter the third shot dates? Are they going to have you resubmit your updated CDC card to get a QR code with extended validity?

We submitted ours on August10. We received the automatic reply, but no QR code, and no further response from the diplomatie office. We arrived in France today, August 19.

based on another forum thread, we went to the Pharmacie Du 58, located at 58 Rue du Oberkampf, near the Parmentier metro stop. 20 minutes later we had the pass sanitaire. Their fee was 20 euros.

I re-submitted the form in French like someone else had done, but otherwise the attachments were the same. I don't think it actually matters whether you submit the French or English form though based on mixed responses. It seems like timing matters most.

I was also thinking about pass expiration since I received my second dose relatively early. I guess we'll just have to cross that bridge when we get there!

I combined all items into a single pdf. In fact I put the copy of the cdc card & passport on the same page.

Interesting. I had mine as separate attachments. Just goes to show you how random this process is!

I combined mine too. I submitted it days ago and haven't received a QR code. We leave tomorrow. I was wondering if that was why. I guess not...

I agree. Every single restaurant that I've been to want it. Some, e.g. Laduree on the Champs not only wanted it but wanted your name and contact info. And in addition, they had the venue code to scan.

At my hotel, they scan you when you come back, but only once a day. Usually someone says that they know you.
