At Barclays, if you already have an online account, the card can also show up there before you get a notification from them. Obviously, that won't work if you don't yet have an online Barclays account from another card. But if it's not your first US card, you presumably do have an online account already.
2) What's the reconsideration line number?
866-408-4064. Only open M>F, 8am>5pm, Eastern.
3) How long after my first purchase does it take for the bonus miles to post in my account?
Miles for the first-purchase bonus post with the statement showing the initial purchase.
4) For those people who got the targeted offer, how long after spending $750 in each of the 3 months does it take for the bonus miles to post in my account?
Miles posts two months after the end of the promotion period. For example, if your promo was to spend in June, July and August, the 15k bonus would post with October statement.
5) How can I get the anniversary 10K bonus points without paying the annual fee?
The annual bonus posts on the statement date of the month of anniversary. No need to "kick out" a statement. But if you are worried, you can put a few bucks on the card during that cycle thus generate a statement but it is NOT necessary.
AF is billed the last business day of the month of the anniversary (much better than Chase practice which is on the 1st day of the month). You have 60 days to cancel the card for the AF to be reversed. You also can use SM to cancel card and no need to call.
Note: I called to cancel the card (June, 2014), told them it was because the AF was too high, and they waived it. Still got the 10,000 points and the Companion Certs!!

2nd card instant approval
1st card was in Aug. Only made 1 purchase. Got the $750 offer Nov Dec Jan but I did not see the offer until almost end of Nov (was in Europe until then) and decided to pass... kind of dumb when I thought back. 
Applied this afternoon using link in first post and left employee info blank. Instant approval with card showed up at account online ready to activate...

Yes. They are listed under the same userid. One is the existing one, the other waiting to be activated. Of course not until the actual card arrives it cannot be activated yet.
Many posters report they are approved the 2nd one when the 1st one remains open. Though majority of such tends to be instant approval. Recon seems to be more an uphill battle but successes are still possible, just not high rate if you carefully follow the thread you will find the trend.
I believe Barclays uses SSN to match just like how Chase does. Therefore the new card shows up with your existing Barclays card. Though with Chase even you are instant approved, the new card still takes a day or 2 to show up but Barclays seems to immediately assign account number and shows it online.

One family member instant online the other one "need more time". Will call reconsideration tomorrow.

Applied my first card yesterday, not instant approval, called into reconsideration line, was told approved. I was transferred to customer service to confirm signup bonus, the rep said my annual fee is not waived. However, my card was fedexed to me today, and paper states the annual fee is waived.
I was charged a $30 fast delivery fee which I did not request. So I called them and they are going to credit the $30 back.

Tedimed - Congrats! And forget US Airways, Star Alliance is awesome! I'd book a trip on my favorite alliance before the US merges with American.
