At Barclays, if you already have an online account, the card can also show up there before you get a notification from them. Obviously, that won't work if you don't yet have an online Barclays account from another card. But if it's not your first US card, you presumably do have an online account already.
2) What's the reconsideration line number?

3) How long after my first purchase does it take for the bonus miles to post in my account?
Miles for the first-purchase bonus post with the statement showing the initial purchase.
4) For those people who got the targeted offer, how long after spending $750 in each of the 3 months does it take for the bonus miles to post in my account?
Miles posts two months after the end of the promotion period. For example, if your promo was to spend in June, July and August, the 15k bonus would post with October statement.
5) How can I get the anniversary 10K bonus points without paying the annual fee?
The annual bonus posts on the statement date of the month of anniversary. No need to "kick out" a statement. But if you are worried, you can put a few bucks on the card during that cycle thus generate a statement but it is NOT necessary.
AF is billed the last business day of the month of the anniversary (much better than Chase practice which is on the 1st day of the month). You have 60 days to cancel the card for the AF to be reversed. You also can use SM to cancel card and no need to call.
Note: I called to cancel the card (June, 2014), told them it was because the AF was too high, and they waived it. Still got the 10,000 points and the Companion Certs!!

I called and it seems that I was not eligible for the 10K anniversary miles. Kind of a bummer. That was the only thing keeping me with using the card. I double checked the info they sent me when I got the card and didn't see anything there either.
I did ask to cancel and was sent to a specialist. I wanted to cancel without the fee. He was actually able to credit my account $23. Not much but I appreciated the effort and took the deal to keep the card for a year.
As a side note: I did get a letter saying I had to cancel before 12/31 or the fee stuck so I decided not to risk it.

Another question about the 10k anniversary miles -- do they show up on the website prior to posting to your US account (like the initial 40k), or do they just show up out of nowhere directly in your US account? Seems to be the latter?

Wife's were on her applicable statement, then posted to US account a day or two later. Mine should be on my Jan statement.

CSR waived $89 fee for me.. was pretty happy with the result.

I got instantly approved through the old link for the world premier card, but with only a 2k limit. How do I tell I actually got the world premier vs the lesser version, as the low limit seems like the lesser card?

I had a 15k offer for the exact same time period: spending $750 in each of August, September and October. However, my December statement date was on the 5th, and the miles did not post.
Since the offer states 6 - 8 weeks, and it has not been 8 weeks yet, I guess I will wait until my January statement to give them a call.

Well, I was feeling uneasy about this because multiple people posted that the bonus was reflected on their December statement, and mine wasn't.
Specifically, there was also a billing error on my statement in October. A large duplicate credit posted during the last month of the promotion. If it had posted once like it was supposed to, then I would have netted the $750 in spending. But since there was an error, and it posted twice, the total was less than $750. This was reconciled in November after the promotion had officially ended for me.
Anyway, a very helpful agent put me on hold to check it out. I was on hold for over 15 minutes. When she came back on the line, she said the credit would post on my January statement date. That will be about 10 weeks after the end of the promotion, but I am still getting the miles! And that is what matters.

Well, I was too optimistic too soon. The 15k bonus miles did not post as promised on my January statement. Looking at my online account, I can see the miles posting history with each stmt, and the number for January on that card was 0. So I called Barklays again, and spoke with another nice agent. Even though the agent I spoke with in December claimed there was a note on my account giving me the 15k miles, the new agent said there was no such note. However, I had written down the date and time of my call in December along with the name of the first agent. The new agent included that information in the case file.
She filed a claim for the 15k miles. I should know in 2 weeks if I get the bonus. Disappointing to have all of these problems after I was told the situation had been resolved.
On a positive note, I had applied for and received a 2nd card in December. Interestingly enough, Barklays assigned the same closing date to the 2nd card as the 1st card. The 40k bonus has already posted from the use of the 2nd card.
