I can only think that it was a new employee since I have checked in there for domestic flights as a OWE many, many times during 2017 and early 2018 (showing my LA and then QF electronic card) and have never had to wait.

Technically on a domestic itinerary, you are the only passenger allowed to use Flagship Checkin as a OWE. If it's international you get a guest. So while not the best experience given she did make an exception for you.

While it doesn't specifically say that no guests are permitted, all of the access rules state "you," not "you and a guest" or anything else that would indicate that anyone other than the status- or ticket-holder has access.

If you use Flagship Check-in at LAX without TSA Pre, do you still have to walk up to the regular security line?

It's been a while since I've checked-in at LAX rather than just connected through, but this applies at all FCI locations: if you don't have PreCheck, you will have to go through the regular security line. TSA and FCI are two wholly separate things.

I used the Flagship check-in last week at LAX and unfortunately, my TSA Pre failed to show up on my boarding pass. I was escorted to the front of the TSA Pre line but had to go through the regular procedures (body scanner and taking out laptops, etc.). There was no one there as it was very late and it was all very fast.

Can we use AA Flagship check-in in T3 if booked in first on BA from Chicago to Heathrow? Ticket was booked via BA, too. Then, we’d clear security at T3 and take transfer bus to T5.
