Official size as per Rule 23.B.2, Baggage, Contract of Carriage:
Under the 2013 contract of carriage, which is still valid for various tickets outstanding, the cabin baggage rule states a maximum of 45 linear inches combined for the LHW dimensions with no given side exceeding 22 inches. For such tickets, the cabin bag sizers are not lawfully applicable if the LHW combined linear inches don't exceed 45 linear inches and no side of the bag exceeds 22 inches. In other words for tickets issued last year, some bags allowed under the contract of carriage don't fit in the sizer without sticking out on at least one side.

Yesterday, UA1707, TPA-IAH there were several announcements pre-boarding about the sizers and the limit on the number of carry-ons. There were at least three announcements stating that if you were in Boarding Group 5 there would be no room for a rollaboard as the flight was full.
I boarded with Group 1 so don't know if anyone checked baggage or not. Towards the end of the boarding process one passenger went towards the back of the aircraft with a rollaboard which was at least 18 inches wide while another had a folding garment bag/luggage, both of which would not have fit the sizers.

Yeah, I routinely handle her bags until we reach the plane itself. We've been stopped once on JAL, they forced a gate-check even when we explained that I was pulling her rollaboard.

Personally, I always carry a CPAP. Rare is the occasion that I am not questioned at the door about why I have three bags. People bringing more than the 2 bags allowed on board are NOT a problem that needs to be addressed unless it is at the individual GA level.

C3 red-coat at EWR was making everyone going through PreCheck lanes with a roller check it in a sizer. Pissing everyone off.

Whopper of the day, brought to you by an EWR GA: "Don't bother trying to squeeze your bag into the sizer. The sizer is the same size as the bins, and if you have to squeeze it into the sizer you'll have to squeeze it into the bins. Pit must be checked."

This is possibly the most well written, accurate post of this very long thread. My hat is off to you.
Not to correct you in any way, but to make an equally valid point, if my laptop bag goes under the seat in front of me, is that not much more important than having it fit in a sizer at the entrance to the jet bridge?
I have found my solution. It's going to cost me about $1000 in new bags that replace perfectly functional bags, one of which fits perfectly into the carry-on sizer. My current carry-on has a shoulder strap, and my laptop case, which is larger than the personal sizer, has wheels. I "can" carry both, but my back would suffer for three to four days after every flight. So I'm buying a new carry-on that has wheels, and a laptop case that will fit the sizer.
All of this is for what, exactly? For me, the decision to buy new bags is based on the fact that I know that I'll run into a GA who will be a "rules are rules" type, giving me a nightmare situation with my laptop, and I will never know when to expect it.
"If" UA figures out how to turn this into a profit source, all other domestic carriers are likely to follow suit. I'm not switching airlines this year, at least, so the $1000 is worth it to me. If OALs pick it up, then I'll be prepared (ostensibly). If not, well I'll have nice new bags that are easy to carry on board.

I hope that your solution works well for you and that you can continue to travel with what you need onboard without having to worry about your whether your computer bag and medical device(s), etc. fit in the "magic sizer".
I only hope that reason will eventually prevail and we can go to a policy where the standard is whether an item fits in the bin in the correct orientation or under the seat.
