Smallest for me is Canouan airport back in 2002. This was before the runway was extended from ~3,500 to 5,500 ft. Here is a picture of the runway:

and here is a picture of the terminal (I do not remember it being an enclosed structure when I was there, they must have upgraded when they extended the runway in 2008)

What is the smallest airport you've been to?
Commercial airport, on a commercial flight.
Mine is PBH. Served by one carrier, Druk air with just three planes.

Been to PBH as well, a great flight from/to Kathmandu. Buddha Air has also been flying to PBH since August 2010.
I'd probably give the credit to SAB. A few Winair flights are day plus occasional charters. Not big enough for jets and the airport itself is smaller than PBH.

Virgin Gorda (VIJ). A small island in the British Virgin Islands served by 8 passenger scheduled charter flights from San Juan. A shack served as the customs and immigration office.

I have been to PBH too. But I think Sukhothai Airport in Thailand (THS) is even smaller. Only served by Bangkok Airways with ATR-72s. The terminal is open air and gets just a couple of flights per day.
LPQ (Luang Prabang, Laos) is also pretty small.

The airport on Kadavu Fiji. The runway was a dirt road and the terminal was a little grass shack.
There are no real roads on the island and so no cars. Had to wade out into the ocean to get to a boat to get to the place we were staying.
Great place - it was paradise and the diving was fantastic!
Oh, I almost forgot. Flew Pan Am Air Bridge from Fort Lauderdale to Bimini. That was cool. It was a sea plane. I loved that!

I guess it depends on the definition of commercial airline and flight.
There is scheduled air service to airstrips, as opposed to airports.
Some in alaska include Old Harbor, Akhiok, Wales, Diomede, maybe Point Hope.
Akutan isn't even an airstrip, but a water strip, served by an amphibious plane that rolls up onto a concrete slab.
In East Africa, the Masai Mara has a bunch of airstrips.
Sukhothai has a largely outdoor airport, as does Lamu in Kenya.

Holiday Island resort in the Maldives, the airport is just a floating platoon in the water and the seaplane arrives and picks you. Its usually not a nonstop flt to MLE because you stop at all the other airports or islands with pax on your flt. Each resort has their own airport.
Other than that, it would be Praslin in the SEZ.
