Same here:

11/10/2010 Continental Debit Bonus 12,500
12/17/2010 Chase Debit Card Enrollment Bonus Miles 2,500

Yet none of my business checking miles have shown up yet. Did confirm it via secure msg but still waiting

I have this card and currently have about 37K miles with Continental (I started off with about 3500). I use this card for all of my expenses for the points. It has been great.

I signed up online for this using a coupon code from the link on Frugal Travel Guy's blog. I was never given an option to request the continental debit card and have just received a regular chase debit card in the mail. Any ideas on how to get the continental debit card and the additional 15k miles that come with it?

You would most likely out of the luck now with this being an afterthought.

For all folks who opened their accounts online, they would get TWO tracking forms - one for the 10K opening bonus, one for the 15K ordering the CO debit card.

Dont know why you do not go to CO OnePass page and DIRECTLY access the most current links right there on CO website, instead of using a link on 3rd party site that may or may NOT get updated.

Good Luck in calling the customer service at the number on the back of your card. Or better, to a branch to try fixing this if you have access to a branch.

Don't know what a "tracking form" is. There was a confirmation after I opened the account for the 10k bonus, but no mention of the 15k. As I said in the first post the online process never gave me an option to choose the continental debit card vs. normal debit card. I would have thought since I signed up through a CO link that it would automatically give me the CO debit card. For others that signed up previously, was there an option during the online process, or was it just automatic?

FYI, linking from the CO site takes me to the EXACT same page as the FTG blog link.

Would love to go to an actual branch and talk to a real person, but none exist anywhere near me, hence the online signup. I called in to request the card and spoke with someone in India. I now have a CO debit card coming to me, but figure I will have an uphill battle to get the 15k.

I ran into the same problem (signed up online using the coupon I got through continental.com, saw no option to select CO debit card nor 15k). I called the next morning and spoke to a rep who said that she would cancel my normal debit card order and send me the CO card and that I would get the 15k miles. Only going on her word for now, but I will hope for the best. I do see on my online account that the CO Rewards debit card is listed. Still haven't gotten anything in the mail except for a letter from Chase begging me to sign up for overdraft protection... :/

When I signed up for the account, I just clicked on debit card, typed in the code, and the CO debit card and my checks arrived at the same time...

You can spend some time to read back some pages to see what others experiences are. Go back to the period of Sept - Oct when the offers were supposedly to expire, there were enough posts about making sure to have TWO confirmations whether online or at the branch - because the bonuses are structured in TWO portions each tied to a different task.

In the pages upthread, there is a specific 800 number mentioned multiple times when people do not see their bonuses, this is THE phone number to call. All else, would not help you to resolve any hiccups because other depts seeem to have no clue on how this works.

It would be to your own benefit to spend some time read back the posts upthread.

I was able to open one personal and two business accounts in 2008, 2009 and 2010. I sure hope this is offered in 2011

I had two Chase checking accounts that I closed last week. Yesterday, I went to a branch and opened a new one. The banker said I was eligible to get the 25K bonus. However, I didn't get any printed confirmation. He gave me back the coupon and said that the coupon number was the tracking number, which is certainly not like what is discussed here and I read only after I came home from the bank. He asked for my OnePass number twice because the computer asked him to enter it in two different places. He verbally confirmed that everything was good. Without printed confirmations, should I be worried? Is there any other way to confirm that I was indeed enrolled in the promotion?

Thank you, I will hunt for the specific 800 number.

Yes - opened a personal account earlier this year (Jan or Feb, I think), and then another in October. Got the full 25k for the first account, and only 15k for the second. I was thinking about calling up and asking about the remaining 10k, but figured that might be pushing my luck as I probably wasn't supposed to get the second 15k either.
