At Barclays, if you already have an online account, the card can also show up there before you get a notification from them. Obviously, that won't work if you don't yet have an online Barclays account from another card. But if it's not your first US card, you presumably do have an online account already.
2) What's the reconsideration line number?
866-408-4064. Only open M>F, 8am>5pm, Eastern.
3) How long after my first purchase does it take for the bonus miles to post in my account?
Miles for the first-purchase bonus post with the statement showing the initial purchase.
4) For those people who got the targeted offer, how long after spending $750 in each of the 3 months does it take for the bonus miles to post in my account?
Miles posts two months after the end of the promotion period. For example, if your promo was to spend in June, July and August, the 15k bonus would post with October statement.
5) How can I get the anniversary 10K bonus points without paying the annual fee?
The annual bonus posts on the statement date of the month of anniversary. No need to "kick out" a statement. But if you are worried, you can put a few bucks on the card during that cycle thus generate a statement but it is NOT necessary.
AF is billed the last business day of the month of the anniversary (much better than Chase practice which is on the 1st day of the month). You have 60 days to cancel the card for the AF to be reversed. You also can use SM to cancel card and no need to call.
Note: I called to cancel the card (June, 2014), told them it was because the AF was too high, and they waived it. Still got the 10,000 points and the Companion Certs!!

Just wondering when I can expect the 10k anniversary bonus. Got approval email on 22 Nov, 2011. Statement closes the 17th of the month so if they bill the annual fee, it will probably be on next month's statement. Should I expect it to hit my USDM account on the 22 Nov, 2012 (exact anniversary) or when next month's statement closes?

Canceled mine and wife's cards with no problems at all after 10k anniversary bonus miles posted (before the annual fee). Hilarious discussion with the rep trying to keep me. I tried to get them to waive annual fee or get another 10k miles. No go. The only thing they would do is give a 25% discount on the annual fee. I proceeded to cancel. Just fyi.

$89 for 10k miles isn't THAT bad of a deal.
Less than a .01 a mile. Way better than the .35 cents a mile to buy.
Makes me consider keeping the card for the 10k, and DMS (5k reduction on redemption).

Well, I canceled after getting the 10K but before getting the AF so I bought miles for 0.00 cents. And I'll reapply for the card soon, giving me access to DMS again.

Just an FYI, a family member, who used her card regularly and probably spent $8 - 10K for the year, got a more generous retention offer than I have seen reported here: a 75% reduction in the annual fee.
In her first call (making clear how much she liked the card but could not pay the AF due to her personal situation), they offered her a 50% reduction of the AF, or two downgrade options (one card with a $49 AF and less benefits and one card with no AF that would only give 1 pt per $2 spend). She told them she needed to think about what she was going to do, and when she called back a few days later to downgrade to the no AF card, they offered her a 75% AF reduction to keep the Premier World MC.
In her first call (making clear how much she liked the card but could not pay the AF due to her personal situation), they offered her a 50% reduction of the AF, or two downgrade options (one card with a $49 AF and less benefits and one card with no AF that would only give 1 pt per $2 spend).
