Nope. The real answer to that question is lurking in one of the earlier posts in this thread:

Well...almost the real answer. Time, reason, initiator's name, supervisor's name, and resolution. Nothing else unless we find something and the only names that go on the form are TSA names, no passenger information.

Seems to be one of those infamous instances of Consistent Inconsistency®, as we have one member of this board who has indicated that said information is captured each time said member attempts a No-ID travel event.

I raised this idea some time ago and still support it.

As for others being inconvenienced, that would be most unfortunate. However, I won't feel too guilty as they often inconvenience me by not being prepared for the TSA screening procedures and by moving slowly through all facets of the airline transport process.

Not an incident report. It's a form looking for generic information - date, time, airline, flight number, originating airport, destination, name of TDC, name of TSO doing the screening. No personal info about the pax. This was addressed previously in the thread.

I think the reson for the info is for bean counters in DC. There is no way for the info to be linked back to the pax.

It was "addressed previously" except that I watched the TDC fill out the form and my name was most definitely on it. I have no idea what the official name of the form is, but it most definitely included my name. I'll try to get a blank one next time I pass through.

The form is supposed to contain no personal information. But given the inconsistency from airport to airport, I am not surprised to hear that it is done differently at other places. I think I mentioned this before, but there are two forms as an option for the TDC SSSS. One form is to contain no personal info; the other form is used in the event of a LEO call, and this form requires more personal info. It is possible that they used the LEO TDC form in your case. Was it a full sheet of paper? The "no personal info" forms are only a half sheet (at my airport).

The forms are marked "Sensitive Security Information" (insert laughter here).

Nowhere was any PAX-identifying information recorded, at least on the form I saw, which as someone else mentioned is for the bean counters back in DC.

However, that doesn't mean some PAX have had their names associated with the completion of this form which would be violations of the Privacy Act as debated in a thread elsewhere (not here please).

I would be curious to know whether this type of "SSI" (insert laughter here) is being funneled into the TIA..ahem..sorry, CAPS...uh oh..CAPSII system to monitor for potentially suspicious behavior.

They churn through the data and determine (ORLY?) whether certain destinations/flights are more prone to PAX flying w/o ID. From there they would (right...would implies proactive) decide to fly more FAMs on those flights in the event of an incident.

*shrug*

I didn't think a blank form could be SSI, just a completed one. Either way, maybe I'll make a stink next time if they write down my name. I'd settle for being allowed to take a digital photo of the form, though I'm sure that wouldn't go over too well either.
