Well...almost the real answer. Time, reason, initiator's name, supervisor's name, and resolution. Nothing else unless we find something and the only names that go on the form are TSA names, no passenger information.
Seems to be one of those infamous instances of Consistent Inconsistency®, as we have one member of this board who has indicated that said information is captured each time said member attempts a No-ID travel event.
