The form is supposed to contain no personal information. But given the inconsistency from airport to airport, I am not surprised to hear that it is done differently at other places. I think I mentioned this before, but there are two forms as an option for the TDC SSSS. One form is to contain no personal info; the other form is used in the event of a LEO call, and this form requires more personal info. It is possible that they used the LEO TDC form in your case. Was it a full sheet of paper? The "no personal info" forms are only a half sheet (at my airport).
