My fave: The 2001 CO bonus machine: CO OnePass flyers could sign up for multiple AMEX Double Miles promos and assorted bonuses that resulted in bonuses as high 5 to 7 times the mileage flown (some bragged of even more!). And it lasted for several months. Great times those were.
Also got in on the $20 British Airways USA-Europe WT+ (premium econ)

gleff seems to have nailed them on the head. If only I'd been on FT when the $20 WT+ runs were about.
There was also the 136GBP LHR-HKG-LHR flights with BA which included nights at a hotel
Unfortunately I've jumped on none of them

Aside from some of those mentioned (namely, the $20+ BA fares), there are also the $2 US fares to ART/LEB, as well as a $59+ SMF-CUN deal that popped up last May or so.
Of course, I'm still hoping that CUN will be rebuilt by the time I go.

Although I booked the infamous US$20 BA T-class NAmerica to Europe fare on Travelocity, the next morning Travelocity emailed to say they could not accept my Canadian credit card, so booking/fare not honoured.
Did luck out on the recent 20 SK C-class OSL-China fare glitch on Expedia.ca and no credit card problem.
And the ACers will remember the acclaimed Mexican Hat Dance, not cheap but lots of miles in business class for less than the old return economy fare of a transcon.

They're a fact of life and the sooner you accept that reality, the more you might appreciate these glitches. Remember, it's the UK and US government who added most of the additional costs to the BA $20 ticket, to the tune of about $40 for the former and $30 for the latter regime. Airlines have little or no control over these charges, nor the soaring cost of fuel that adds those pesky fuel surcharges.

Some of us were scoring sextuple status miles.
I had one same day turn around to SAN that scored 38K status miles and 58K total miles. All for $178. Just wish I had done a few more of those.
