USB Buxx
(NOTE: stopped taking new applications ~mid February 2014 through the official link.

Also - Answer ALL the identification questions in your profile (not JUST three or four) USBuxx will sometimes ask you one of the questions, if you use the account on a new computer. HOWEVER - they are more than happy to ask you an unanswered question - talk about BROKEN! So, answer ALL the questions so that when you are prompted - you can answer any of them.
Do not load with any US Bank card as this will not provide points.

Max # of CC on file for both is 2. Each has a max # of loads per card per 7 day rolling period (~3/4). If you card fails to load, first try to switch to another CC, second check to make sure there are at least 7 calendar days between loads.
Reports of Citi cards (at least Mastercard) coding as purchase for NW Buxx (Jan 2015), but YMMV.

Do not create too many accounts too fast. Some reports of account shutdown when same SSN used across multiple cards / accounts in a short period of time.

I had this problem with my first Buxx account. I funded it with a credit card that has stored my address uniquely. Unfortunately I didn't realize that I could not change my Buxx address for 90 days (so said rep), so when I created the account I input the address in Buxx to match the crazy funding card address.
So now I'm stuck using just this one card to fund 3 Buxx cards because I don't want to go and change all my other cards to have my address match my Buxx account, which I purposely screwed up to match the one CC.
I'm highly frustrated by the catch-22 and can't wait for the 90 days to be done.

Anyone still using the NW Visa Credit card to load to NW Buxx cards? If so, does that count as a purchase an still give points/cashback?
The NW Visa CC has 1% cash back and there will be no load fees for loading onto the NW Buxx card (so effectively 1.4% cashback, since you save $4/$1000 load)

New USB Buxx CC changing limit
Just notice new limit below on changing CC at USB, which effectively limits # of CC that can be used to 3, and the max CC load in a month.
- add/edit 2 credit payment methods every 24 hours, 3 every 30 days, and 5 every 180 days.

So... I was able to get a TD Go card by changing my billing address at Capital One....
Downside? You won't be able to add a card with a billing address outside the allowed. So... Pretty useless now.

This is exactly what happened to me with my USB Buxx! Rep said I could change USB Buxx address only after 90 days. But until then I'm stuck using just that one credit card with the goofy address.
