Serious question: What makes a "luxury" hotel?

I've just realised that I've probably never stayed at a true luxury hotel. I usually stay at 4* properties or down to 3* when in a very expensive location (London, Paris) or able to get a suite for the price of a standard 4* room. Of course, I've been to 5-star properties but they were big hotels, and I have a subjective feeling that they wouldn't really be classed as luxury hotels by the regulars of this board. TBH the two most recent ones didn't strike me as being any better than a regular 4* - the suite at the Radisson SHJ was big but not that well-furnished (admittedly the staff were uniformly good), and the personnel at Le Meridien BUD came across as conceited, unhelpful and not very knowledgeable.
So, what is your definition of a luxury hotel? How does it differ from a regular 4* or 5* one? Aside from the usually (proportionately) bigger public areas and slightly more elegant furniture, what are the extra amenities/services one should reasonably expect to receive at a true luxury establishment over your typical 4 star hotel?
I'm more interested in your opinion of city hotels, as resorts tend to offer things that are easier to process/understand for my limited brainpower (e.g. massive gardens, pools, private beaches, restaurants in fantastic natural settings and so on).

I know this has been a subject of controversy on this board, but I would define a luxury hotel in a major city as having the following characteristics:
- 24 hour concierge service (a team of multilingual clef d'or concierges)
- 24 hour in room dining (quality food delivered 24 hours a day in a timely manner)
- 24 hour security
- One hour pressing, shoe shine, etc
- 24 hour laundry and dry cleaning services
- 24 hour doorman and valet, hotel has luxury car services
- good quality boutique or hotel shop (not tacky, some luxury hotels do though)
- a gourmet restaurant (ie. the restaurant is a step up from the other restaurants in hotels around the city)
- a marble bathroom with separate shower and bath (i know this can vary)
- an elegant room with expensive linens, quality bed, quality in-room technology, room size (above average)
- nice public areas (nice bar, elegant lobby, flower arrangements, etc)
- a large expansive spa (not all luxury hotels have these) with a pool (not always there)
- good housekeeping (24 hours a day, twice daily, keeping the rooms well maintained and clean, putting a bookmark in a guest's book, folding and hanging clothes, etc)
- attention to detail, customer service focused, well responsive to guests needs, remembers past guests, takes notes of guest preferences
- property is renovated recently (ie. the property is concerned about maintaining its position, not resting on its laurels)
- quality reservation support
- a good central location in a safe and clean area

Example of hotel companies: Four Seasons, Peninsula, Mandarin Oriental, Ritz-Carlton (somewhat of a mixed bag), etc

Specific Hotel example: Four Seasons Hotel George V, Peninsula Hotel Hong Kong

These are just a few of the things I use to judge a luxury hotel.

For me, a luxury hotel is one which operates around my schedule and convenience and not the other way around, to a large degree, and within reason. I would not expect a luxury hotel to open up their hotel restaurant at 3am just because I want to eat there; but I would expect a breakfast delivered through room service to my room at 3am.

To comment on Nachria's ideas a bit:

Really great concierges. I don't personally feel they need to be there 24 hours a day. Part of the reason is, though, that I like it best when there are only a few (who get to know you rather than there being like 10), and it would be logistically impossible for a hotel to have a small team of concierges and have them there 24/7.

24 hour security - most definitely. I DON'T want to walk down 14 flights of stairs b/c somebody pulls the fire alarm at 2am to be told the next morning that "they don't know who it was and they don't keep track of who enters the hotel." Hint, hint, Park Hyatt Philadelphia.

Boutique - I don't really care if they have one or not, but if they do, it should be SMALL and NOT be tacky.

Bathrooms - let's not be too rigid...there is no need for marble. Wood and porcelain and some trendier stones often works well in modern places.

Public areas - these should be really nice and have places to sit BESIDES the bar.

Housekeeping - should be really good and neat but should NOT rearrange anything.

Now, for some of my ideas:

Good recovery service - hotels - even luxury hotels - make mistakes surprisingly frequently. These hotels need to a) fix it really quickly, b) do something to figure out how to make up for it. The effectiveness of solving the situation is what defines the best hotels.

Generosity - the best hotels should be generous and should not nickel and dime. To me, this often separates the best luxury places from the less great ones.

Size - luxury hotels really can't be too big. Otherwise, they lose the intimacy.

Edited to add: I really like luxury's description. A place that works around your schedule, not the other way around. Totally true.

Interesting thread: One mentions great public spaces.. When Cesar Ritz developed the Ritz in Paris, he specifically did not want a comfortable lobby. Having stayed there, I can say that there are no welcoming or comfortable seats at the Ritz in Paris. Hotel DuCap in Antibes also does not have a great public area, nor does the Gritti in VCE. One is now more likely to find good "public spaces" in the newer & chain hotels. I have stayed in many luxury hotels and the difference seems to be all about what the concierge can do. In Paris, the Concierge at the Ritz directed me to a salon which could fix a broken fingernail. Yet, at another stay at a 4* hotel (Hotel Bel-Ami) in Paris managed to find a shoe cobbler. The heel on my one nice pair of high heeled shoes broke one hour before our lunch at Taillevent. I think that "luxury" is highly subjective. My favorite hotel in VCE is 4 Star: If it had a restaurant, it would b e categorized as 5 star. My VCE hotel has 24 hour room service, Clef d'ors Concierges and is, in my mind, luxurious. It is not luxurious in the sense of the Peninsula, in HKG, where someone jumps out of a closet to attend to your every need. Some times a Four Star Hotel can be a more luxurious and personal experience than a 5 star Hotel.

Come to think of it, very few European hotels are designed with inviting public spaces for sitting around. So I guess I should maybe scratch that from my list. I wasn't thinking about European places when I wrote it; I was sort of imaging an Asian hotel.

I'd say the big key is service. GREAT concierges, intuitive servers, great maid service, etc. 4 star hotels can have great rooms, but they often lack the service.

And obscure2k, what hotel is that in VCE? I might consider it next time I'm there.

I would add a high ratio of staff to guests. Once the staff makes an observation about your preferences, they anticipate your needs. One of the best vacations we ever had was at the Four Seasons in Nevis -- they noticed little things and made adjustments.

The main thing that distinguishes a luxury hotel from an upscale property to me is service. When I am in a really good hotel, I just have the feeling that any request within reason (and many not within reason) will not only be done, but done with style. They've even anticipated my needs -- sorry, I expected warmer weather, so I didn't bring an overcoat: No problem, we have one to loan you!

Luxury is hard to define but when a hotel has it, it's obvious. Service is important. They must make you feel important and treasured as a guest. Rooms must be immaculate and turn down is a must. I like having one button on the phone to a concierge that will take care of all requests whether it's room service, reservations or a wake up call. I do not like being transfered or put on hold. Bathrooms must sparkle. I should always be tempted for an evening bath. Shower must have strong water pressure and hot enough to adjust down. I should never be awakened at 8am in the morning by housekeeping. Through some divine power they should actually know when I'm not in the room and service should be so efficient that they are never in the room when I return. Pool must have food and beverage service. A spa is a necessity. At least one Michelin star quality restaurant in the house. I know some of my needs are too difficult to achieve all but these are the things I notice when staying at a hotel.

And by the way, do we need to come up with a new term for "luxury"? Because everyone throws around the terms luxury and 5-star with no regard for their true meaning. I don't blame the OP for being confused.
