The 6/21/11 sale distances were 450/1250/1250+
Seems like we have a pretty clear idea of when they have been releasing these sales. One in early summer and one in October. Also of note, the summer sales are valid for travel from mid-August until before Thanksgiving. The October sales are for post-Thanksgiving to mid-February, excluding Christmas/New Years of course.

The October 2012 sale was not distance based. It was 40% off using a promo code, which pissed people off because they couldn't use points.

Last year there was a big sale on October 8. We may get another one on Tuesday October 14, 2014. Check around midnight Central Time for low fares in your favorite markets.

There was a sale starting February 8th with $59 fares. I recall it was a pretty good sale but not specifically a mileage sale. The Email links to a webpage with the details, which of course is no longer available (404).
