AL - WN used to fly HSV-BHM and I think the flight continued on to Mobile or Montgomery.
OH - CLE used to be an actual hub for Continental and I think they had some intra-state flights.
KY - Similarly, CVG was a Delta hub and they had a pretty extensive regional network via Comair.

just last month I did SEA<>GEG on the retirement flight of the Alaska (Horizon) Dash 8-400

No one serves VIS anymore, they opted to take a cash payment from the federal government in lieu of EAS service. They have a 6x daily shuttle service to FAT, which is probably sufficient.

This is interesting - I think my list is as follows:
  • Texas - AUS-DFW, AUS-IAH, AUS-HOU
  • Georgia - SAV-ATL
  • Florida - MIA-MCO
  • Hawaii - KOA-HNL
  • California - LAX-SFO

Several people have mentioned flights from VA airports to IAD. However, DCA is actually in Virginia as well. I've done RIC to IAD, ORF to IAD, and ORF to DCA.

DL served Wilmington, Delaware in the mid-2000s, in part so they could advertise themselves as "serving all 50 states."

Allegheny served ILG the same era as Eastern. I remember the two check in areas. According to wikipedia AA and TWA were the first airlines to serve ILG which I find amazing.
