I think they've gotten rid of it. In any case, it would only be collected/checked if you were on a flight where a case was detected.

What about travel by train from Spain to France?

All the documentations refer to travel by plane sometimes by ship, even the EU PLF only has option for airline..

Do we need to prepare any paper work for France? We will have been in Barcelona for 5 days before traveling to Paris, originating from US.

I'm traveling from the US to France - do I need to fill out the eOS in addition to the PLF? It seems to be asking for the same information. Thanks!

I have the same question about whether we need both the euPLF and the eOS form - am leaving for France on Tuesday. Plus, on the eOS form, it asks if you have any symptoms of Covid, but doesn't provide an option to say "no". Just provides options for "asymptomatic" and "I don't know", in addition to several yes answers. Has anyone had to complete this form? How did you answer the Covid question?

I don't know what is the eOS form ? is that the sworn statement ? If yes, don't bother filling it and the PLF. No one is requesting them.

Hi - it's this:

The French Ministry website with guidance on traveling says that persons coming from orange countries (which the US is) need to fill this out in addition to the PLF. I ran into the same issue as ORDExplorer above - I don't want to inadvertently declare that I have Covid!

I have never seen this before (and I returned from the US a few days ago). I am sure this is certainly not required (and it's weird as it is written "dpistage" on the 1st screen. IMO don't fill it.

Definitely, but as said before, this statement is never asked by any staff and never collected. So no need to waste time and paper.

I get asked for it every time I have to check in with an agent in the US as it is listed in TIMATIC and I proudly display the copy I signed for my initial trip last July. I even point out the date I signed it. I then proceed to tell them that nobody cares about it and the usual response is "Why?".

I am now in France and I can confirm that no one asked to see either the PLF or the eOS at LAX or CDG (I took a paper copy of the eOS with me per pascalgr's recommendation). Thank you to everyone for weighing in!

That form had to be filled for UK-originating passengers during the period of reinforced control due to omicron. AFAIK, this is no longer required for vaccinated individuals (perhaps non-vaccinated passengers from an orange country must still do it?).

i was asked if I had the form when I checked in at Sydney a few weeks ago. I said no, they kindly gave me a copy to complete before arrival in France. The incompltes form is still sitting in the bottom of my bag.
in fact when I arrived in Paris all I was asked for was my passport, at that stage they required a negative test as well, but I was given a very warm welcome and sent along my way.
