i was asked if I had the form when I checked in at Sydney a few weeks ago. I said no, they kindly gave me a copy to complete before arrival in France. The incomplètes form is still sitting in the bottom of my bag.
in fact when I arrived in Paris all I was asked for was my passport, at that stage they required a negative test as well, but I was given a very warm welcome and sent along my way.
