As others have mentioned, In n Out on the West Coast.
Whataburger in the south.
Ritters Frozen Custard in central Indiana.
Another vote for Graeters Ice Cream in Cincinnati.
Hackneys in the Chicago area (for their burgers and their onion loaf).
Somebody mentioned Clancy's...that's a name I haven't heard in years!

In the Chinatown Dixon St. Mall, just in from the corner of Hay St., across from a pub with a TAB parlour, is the best Chinese bakery I have ever had the pleasure to patronize. It's packed with customers every open moment, and the little flaky baked roast pork buns - $0.90 each! - are to die for. We'd buy a bag, and bring them across the street to the pub, and enjoy them with a pint of V.B. Absolutely da kine! So ono, they broke da mouth! I couldn't get enough of them, and bought day passes on the monorail just so we could patronize that store from the Grace Hotel on York & King, easily and often!
That bakery - and Sydney ^ - is definitely on my list of "Places to which I would travel just for the food."

In North Carolina: CookOut - they have 30-some flavors of amazing milkshakes (including eggnog shakes in December and watermelon shakes in July!), and great cheap burgers.
In New Mexico: I echo ButIsItArt on Dion's. Also, I grew up on Blake's Lotaburger - amazing green chile cheeseburgers and fries, and they write your order directly on the bag they put it in, so it's guaranteed to be accurate.

Fatburger has great burgers too. IMO, far better (and more expensive) than In-N-Out Burger.
I might get flamed for this, but in Phoenix Corleones makes the best cheesesteaks outside of Philly. They close early on weeknights, are open very late on Friday and Saturday night, but they're made quickly, piping hot, and always tasty.
O'Brien's is a pretty decent little sandwich shop in Ireland - they even have a few international locations including the Singapore airport. They also have a great chai latte if that's your thing.
I forget the name, but there is a chain mini-grocery + fresh food store in Germany (or at least in Bavaria) that has a daily menu (hot food) plus I think sandwiches. It's certainly a quick and convenient way to get lunch. There's one about three blocks from Marienplatz on Theatinerstrasse very close to Odeonsplatz. I think the name starts with a "V". Maybe "Venzinmurr"?

As long as we're going down that avenue... there's a chain of gas stations/mini marts in Western PA (and now into WV, OH, VA and NC) called Sheetz. Sheetz feature a MTO (Made-to-order) menu. I like their subs better than Subway (no comparison IMHO). You order with a touch screen, so if you screw it up, it's your own fault. They also have a huge choice of fountain drinks and slushees.
Now, the Eastern PA folks will now come out and talk about WaWa. I've only been in them once, and they seem to be similar to Sheetz.
