The requirements for the application process varies from bank to bank based on various online data points collected from FlyerTalk, Doctor of Credit, etc.
Some banks require that you go into a branch and apply personally. Other banks want you to have another account open for 90 days before you can get this card. Some banks (if you already have a relationship with them) let you apply via mail. Sometimes you have to be a resident of the state they operate in, while some were able to get the card without living in those states. It is very much YMMV. You will need proof of income (W2, check stubs, etc). The only states City National Bank operates in are: NY, DE (only trust office, no go), DC, GA, TN, MN, NV & CA. The bank is looking for high net worth individuals, but that's not exactly consistent either.
The card has no foreign transactions fees and is fee-free the first year, then $400 annually.
Register your card for online access (cno.cnb.com). This changed on Sept 19 2019.

I saw the email with the survey, but haven’t taken it yet. I did speak to my banker last week, who indicated that the Crystal card is no longer one of their products that they are focusing heavily on. I suppose that the fact that they sent a survey out means that they are considering some changes. Let’s hope it’s for the better.

It's letting you take the survey multiple time so, if you have a strong feeling about anything, guess you can "weight" the responses:

I took the survey and it seem like they are trying to figure out what benefits to remove and are thinking about linking benefits and/or point to having $100k+ on deposit in linked accounts.

I got the same survey and was then invited to join the "panel" which I declined. I was crystal clear on how I viewed the gutting of the card benefits and the generic (and pricey) wealth management program.

Well, the end of an era for me -- my relationship manager wouldn't do anything about the upcoming annual fee, so I closed the account. When they gutted the card it was clear this was no longer a product they wanted to invest in.
I mostly wanted to thank everyone on this thread for all of the info everyone has posted about this card over the years - I got SO much value out of it because of the tips people shared around redemption. Kills me that I had to redeem points for Amazon gift cards (instead of the trip to Greenland I had not yet convinced my wife to take), but c'est la vie (let's hope that's my biggest covid hardship). Anyway - just wanted to say thanks...onward to the next card.

For a net of $50, I will likely keep the card for another year. I have a ton of points that I can not burn before the card renews, and would rather save them for another year so that I can do a high-end trip that I had hoped to take next summer, but will hold off until COVID vaccines are available.

I'm finally joining the hoard of people that have dumped this card. I called my relationship manager the other day, and she wanted to know the reasons so I explained them (per-card travel reimbursement, no priority pass guests, Visa Discount Air benefit). She understood, and also gave me the soft sales pitch on their asset management services. I explained (again) that I'm a very happy Schwab customer, and have since picked up a Schwab Amex Platinum card (among others) and their benefits are just better than CNB. She was very polite, recognized that they removed a lot of valuable benefits, and was sorry to lose me as a customer. Maybe if they bring back some compelling benefits, then I'd come back, but as it is, my account is now closed.

I don't imagine many folks care anymore, but for those few still left, looks like there's bee a slight change in the physical card design. Had to replaced a delaminating card and the replacement no is no longer "reflective." it just totally a matte finish. It's is also slightly less hefty overall.
However, card now feature edge to edge interior "metal." So now, unlike the old cards that only had metal "cores' that stopped a few cm from the edges, these should not fall apart as quickly as they had in the past.
