Some banks (if you already have a relationship with them) let you apply via mail. Sometimes you have to be a resident of the state they operate in, while some were able to get the card without living in those states. It is very much YMMV. You will need proof of income (W2, check stubs, etc). The only states City National Bank operates in are: NY, DE (only trust office, no go), DC, GA, TN, MN, NV & CA. The bank is looking for high net worth individuals, but that's not exactly consistent either.
The card has no foreign transactions fees and is fee-free the first year, then $400 annually.

Also, I got my $350 GCs reimbursed in February even though I downgraded in January. I found out when my $350 autopayment did not go through. When I called to make sure I didn't still owe the $350, they told me it had been reimbursed and credited back to my account.

I downgraded simply because it's no annual fee and provides the opportunity to upgrade back to Crystal without a 5/24 hit if they make it worthwhile again at some point in the future.
Also, I got my $350 GCs reimbursed in February even though I downgraded in January. I found out when my $350 autopayment did not go through. When I called to make sure I didn't still owe the $350, they told me it had been reimbursed and credited back to my account.

I'm in the same boat. Made a $350 purchase on 1/1 and still nothing. I'm waiting until 2/26 ish to start getting real antsy about it. That'll be the 8 week mark, and 6-8 weeks is what their fine print says for reimbursement.

Primary card purchasing AA gift card: 3 X $100 and 1X $50, purchased on 1/4/2020; posted on 1/6/2020
AA voided all gift card purchase and made a full refund of $350 on 1/15/2020 (Dont know what is going on)
CNB crystal rebated 3 X $100 and 1X $50 on 2/5/2020

Were there any issue with your points transferring to your new account? My Card renewed in December, so I still have time before the next annual fee hits, but can't possibly spend all of the points I earned by the end of the year, and would rather save them for travel versus a gift card or statement credit.

I only had about 1K points left as I spent them on flights last summer so I’m not that bothered either way. Since you can earn points on the no-fee card, I assume you can redeem them.
