Call the UA ICC. Enter your MP number. Enter the wrong zip code twice to get an agent. Give the correct zip code when the agent asks to verify. Tell the agent you would like to apply for the UA Visa card. The agent will transfer you to Chase to fill out the app. When the Chase agent gets on, they will offer you 50,000 miles after your first purchase, and annual fee waived for the first year.
