There are automated gates on entry from landside, and staff at transfers who will send you back if trying to enter the wrong terminal.
The exception is for those who use the personal shopper service.

At LHR, you need a boarding pass for a flight from that terminal to be allowed into the security screening area.

Yes. If you want to spend £250 on something that isn't available in your terminal, you can be escorted to another terminal with a personal shopper. There's no guarantee they will let you go into the lounge - you might just get brought back once the purchase is complete.

If you are connecting at LHR you can ride around the airside transfer buses to your hearts content - no one will check you bp. At T5 if you want to go through connections to the airside departure area you do need a bp for a flight departing T5 - for other terminals this isn’t the case. However I would not recommend doing it unless you are really familiar with what you are doing and the airport layout.

I don't think the T4 PP arrivals lounge offers made-to-order food any longer, sadly. Have used it three times in the last couple of months at various times of day and there has been a small buffet out with no more food available. Sad, as was indeed an exceptional arrivals lounge. Haven't used T2 PP arrivals for a month or so but that did still have a hot menu available for order the last time I used it, albeit the quality of food wasn't wonderful.

So with intra-terminal hopping out, back to my original question - if I cleared immigration to check out PLAZA PREMIUM LOUNGE in arrivals for the shower, would I be allowed in the airside PLAZA PREMIUM LOUNGE right after? Technically should work.? Any recent experiences?

Stopped into the No 1 lounge since the Virgin Clubhouse didn’t open until 630 despite our 5 am arrival. Seems like a nice set up, lots of space, and a nice food options with a buffet and a made to order menu. Didn’t actually try anything since the VS lounge is amazing

LHR Terminal 4 sees yet another lounge option, in the form of the Art & Lounge, which is actually the former EL AL King David Lounge. Opening hours at this time are tuned to EL AL flight departures (06:00-11:00, 15:00-18:00).
