I purchased 10/24, and am still getting the runaround as well. Very frustrating. I've contacted them every week, and now they're just falling back on the "please wait 8 weeks" line. So come 12/18 we'll see what their story is.
I'd suggest doing this via email, I'm keeping a paper trail as I'm thinking this will get ugly. Also the phone support is atrocious.
My communication is with "Emily" who I'm almost certain is a different person every time who simply signs the email "Emily" to sound like it is personal. Grammer style, and zero recollection of the issue lead me to believe this is a pool of CSRs who just take the issue and reply with useless info.

The long nightmare is over...
Unsure if my email last week to [email protected] made the difference, but finally today the miles for officemax posted with a 10/24 date. This was to be my 16th & final hit, just hoping it makes it in the next sweep.
An email sent yesterday to officemax may have also helped... but I doubt it. GL all.

Got a very odd letter with a check from American Express, regarding a $66 Office Max purchase last September. The check was for $32. It said sorry we couldn't issue your Membership Rewards points for your OfficeMax purchase. And the weirder thing, it was an SPG Amex. Thanks Amex.
