Star Alliance retroactive credit question

Hi there --

I flew on US/HP flights last weekend, and despite my giving US my MP number *three times* (once on phone to CS prior to flying, once at gate on outbound trip after rebooking due to flight delays, once at gate on return trip after MP# no longer showed up on boarding passes), at least one if not three of my segments didn't get credited to my MP acct.

On ual.com, it says, "To request retroactive credit for your flight activity on a Star Alliance member airline or Other Airline Partner, send a written request with your name (as listed on your Mileage Plus account), Mileage Plus account number, a copy of your ticket or E-Ticket receipt and your original boarding pass(es) to the Mileage Plus Service Center at the address listed below." The problem is, I don't seem to have a copy of my ticket or e-ticket receipt. I tried to look it up online on usairways.com and the PRN seems to be completely gone since it's past the flight date. I never got an email about receipt because my b/f booked this ticket for me (and I think that when he did, the US/HP system didn't even send him one -- they are so hosed over there).

I have original boarding passes -- if I send/fax those in, is that enough to get credit? If not, how do I go about getting a copy of the ticket?

Thanks.

I do not know if this works for US/HP (it works for UA), but they need your ticket number to give the credit (the one that begins with 016). Since you don't have the receipt....You should be able to find the ticket number on the statement of the chargecard that was used to purchase the ticket.

Ticket number

Your ticket number should also be on the bottom of your US boarding pass as well.

It's actually not. Only the PRN is on there.
