If she truly is that large, can she afford two economy seats?
If not, I would attempt to book on flights w/low passenger loads so shell have a spare seat next to her.

The answer is two economy seats. On many a/c, the business class seat is not much wider than an econ seat -- on a 767, for example, econ is 7-across, business is 6-across.
I'd recommend AA because of their better seat pitch (distance between rows), as well.

You often can get a business class ticket from a consolidator for around $1400, but I agree that 2 coach would probably be better.
In the U.S., I think there was a lawsuit involving this. I don't know how it came out, but if she was proactive, polite, and flexible she might find that the airline would work with her.

Interesting "first" post....
Unfortunately LHR-JFK is one of the heaviest traffic routes and the chances are very slim (no pun intended) of an empty seat next to her being available so dont count on it that.
She should defintely, 100% pay for two seats and make sure the airline knows in advance why she needs two seats.

Ok - I'll post the insensitive answer:
Perhaps she should deal with what ails her before embarking on what will be a stressful and difficult journey.
After all, how much fun will she have visiting New York if she is "enourmously fat" or "extremely fat"??
I'm not suggesting she stay home until she looks like a heroin-junkie fashion model, but she will no doubt be more comfortable on the plane (and in New York) if she is only "morbidly obese" or "extra-large plus size" or "Today's Woman."
And the $800 - $1,000 she saves from buying the two coach seats will help with the inevitable medical bills associated with slimming down.
Perhaps she should deal with what ails her before embarking on what will be a stressful and difficult journey.

Another option is she is as big as she sounds is buying 3 seats together, that would most likely still be cheaper than on F seat.
Plus it wouldn't prevent someone from being stuck (no punn intended) in a window seat

I don't think anyone is too fat for two coach seats. Coach seats are about 17" wide, so if you had a 34" diameter, that's a 105" waist, and I estimate that would require a body weight of 900 pounds.
Anyway, two seats in the outer section of a 767 or a 2-5-2 configured 777 should work just fine.
