A bit of an old article (4 years ago), but it was all over the major networks. In my personal circle (excluding FT), I know multiple people who have the same passport, and I am sure among FTers there will be more than a handful who could have the same claim. Interesting to see the press he received.
That said, my old one is 144 pages (48 + 4 extensions of 24 pages each)

Last year I was departing Dhaka, and the Indian businessman in front of me put his passport in the security bin. It was very worn...and over 1 inch thick. At first I thought it was simply one of those mini Bibles or Quran, but then I saw the cover. I joked with him that that was the thickest passport I've ever seen. He said he travels internationally a couple times a week. I don't know how they got all those pages in there, or why the Indian government doesn't issue a new passport after a certain point.
Regardless, I kick myself for not asking the gentleman if I could take a photo. I try to fly politely, but sometimes I second guess myself.

Just announced this morning, no more supplemantal pages issued after Jan 2016. new passports issued overseas will have 52 pages. no more additions, when its full its full and you will have to get a new passport (or a second passport if you qualify).

The person has some history of affiliation with the State Department too. The additional page insert orders are going to be gutted by or before 2016
My primary ordinary US passport is going to go in for two inserts as soon as possible over this year and well before the fourth quarter of next year.
The Indian government attached old passports with valid visas to new passports. Quite distinct from page insertion.

140 countries have signed onto the push to cap passport validity to five years. Most of those that have reduced their validity period down to no more the five years don't allow for adding pages and those that did have all stopped it in so far as I've not seen them allow it anymore. The US has maintained a ten year general validity period for ordinary adult passports and doesn't seem as likely to kill that off than it is to kill of the additional page inserts -- but neither is scheduled for elimination even as the U.S. is an advocate against other countries allowing for page inserts or allowing for passport validity periods to be greater than 5 years. It's rather amusing when what's good for the goose is not considered good for the gander.
It used to be more common to see multiple passport books bound together than to see additional passport page inserts in non-US passports. It probably still is, for the latter seems to have largely dropped off the radar.
