Real old old-school etiquette calls for the man to get on first, not the woman, but for the woman to exist first. Why? Because getting on an elevator was supposed to be potentially dangerous, and therefore, the man should take the risk first, or remain in the risky situation last.

The same rationale once applied to stairs. A man was to position himself so that if the woman fell, he would be able to catch her.
If a man and woman are entering the elevator and there is no one else in the elevator then the woman should NEVER go in first, if she did/does she would be extremely vulnerable to the man entering behind her.

+1...for the most part.

As often as not, I'd just as soon wait for the next car. Sometimes, however, elevators are slow in coming, I'm running late, etc., so I'll "reach in". I don't consider it rude or inconsiderate....whether I'm already on or tying to get in. (And yes, I make it a point to hold the door open if I'm already on and am aware of someone else wanting to get on.)

Unless the elevator is completely full, it is part and parcel of using one.

I can't stand the colleagues who are having a chat in the elevator. Colleague #1 gets off at floor 11 but holds the door open to finish his chat to colleague #2 who is getting of at floor 12.

Yeah, if the man goes in first he might be in danger of the woman attacking him! The world is a scary place in't it?
