Must Have Accessories
I've got a few of my own must have accessories, including one of those neck pillows, some eye shades (for when hotel windows are up against a parking lot light that you just can't shut out), and a multi-plug extension cord for when you need to charge your accessories with only one outlet (laptop, ipad, phone,etc.)
What are some of your own must have accessories for when traveling? Anything off the beaten path that you've found invaluable? Sound off! Here's my list to start things off:
1. Neck/head pillow
2. Eye shades
3. Extension cord
4. DC car charger (USB plug - when my phone acts as a GPS)
5. Emergency first aid kit - stripped down to aspirin, band-aids and antacid

If you travel to a lot of different countries I would suggest a plug adaptor with multiple inputs and outputs. Oh, and an iPod and a good set of earphones or a good headset.

A good Swiss Army knife, but only in your checked luggage.
Alcohol wipes or alcohol gel in a small 3oz. container is useful, and a good idea.
The old German method I picked up from a multitude of LH flights, was to carry a small bottle of 4711 spray cologne around as a travel sanitizer. This is what used to be given to the mariners on U-boats, rather than showering.
The high alcohol content worked very well, but you end up smelling like Limoncello.
Alcohol wipes or alcohol gel in a small 3oz. container is useful, and a good idea.

Eye shades and ear plugs for flights where you want/need to sleep. Sleep aid helps (couple beers pre-flight and a couple Nyquil tends to do the trick for me).
Agree with the adaptors! ^ Even if heading to a place with like power plugs, you never know when plans change and where you'll end up.
Excedrin Migraine -- cures the headaches from work and the extra glasses of wine after work.
Bottled water -- I always pick up at least a liter after security. The dry atmosphere in the planes plus the aforementioned pre-flight beers do not mix well with proper hydration.
And the most important accessory is your photo ID! You can buy nearly everything else where you are headed, most of the time, but will get nowhere without that.

I keep a travel blanket (which I got from JetBlue for 5 bucks) in my carry-on. Been stuck at the airport enough times to find it invaluable. It's been used as a blanket, bed, pillow, sweater and even umbrella!
Also, my "first aid kit," including safety pins, breath mints and Alka Seltzer.

Fix'd. There are a number of these pocket power units that will work but this is the one I happen to have. This IOGEAR pack is also good and comes with a lot of connector adapters.

The fact that you mentioned those two items together, does that mean the nail clippers to cut the cable ties down to length once you've used them?
I'm very curious...what unexpected circumstances arise where you find the spare cable ties useful?

That'll be to secure suitcase zips (easy to see tampering). Nail clippers are to cut them, but you can't carry nail scissors in hand baggage.
For a little extra tamper-evidence, try coloured cable ties (less likely that they will have the matching colour).
