LAX to KOA
Already booked BA LHR to LAX using Amex 2 for 1
Want to book onwards flight to KOA. I guess best to choose AA as it is part of one world.
Will our BA Gold give lounge access at LAX and or KOA?
Should we book through AA website or can we book through BA?
Finally any suggestions for rental car with full insurance from KOA

BA Gold will get you into Qantas first lounge as well as AA Flagship lounge in LAX. Qantas has nice table service but no view. Flagship has better views and ok buffet. I dont think theres a Oneworld lounge in KOA.
Youll have to book with AA as you cant book a purely domestic US trip on BA.

KOA is mostly open air as it’s Hawaii
there isn’t a lounge there. You go to and leave the planes by walking on the runway
But it’s Hawaii and it’s beautiful
we use Avis for cars but all major rentals are there

Paul611 , note that oneworld member AS (Alaska Airlines) also offers nonstop service between LAX and KOA.
Are you planning to fly First Class or Economy between California and Hawaii? If Economy, note that your BA status would get you free Main Cabin Extra (extra-leg-room) seats and free checked baggage on AA -- even if you were to book AA's cheaper "Basic Economy" fare.

But, KOA is also on the hot and dry side of Hawaii, so being open air isn't always great when you're standing around in the sun waiting to board.

Just to mention, if you go via DFW you will get a full long haul plane with lie flat business seats rather than the "normal American domestic seating" - they have two flights a day to HNL and one to KOA from memory.
At this point it's probably not worth going backwards/forwards, but it is a far nicer experience, and DFW has a good flagship lounge with a nice view of the airfield.
