Here's a contact with a great CSR in AT&T's Customer Rewards/Complaint Resolution Dept.: Maureen, 800-901-4343, Ext. 52206.
She fixed my Marriott Rewards problem (including retroactive credit) with no argument and a friendly, professional attitude.

6k miles earned in just under 6 months, just as advertised.
I've now switched to Sprint with the UA 10k offer - hopefully it will be just as easy.

***Very Important*** If you have problems with your bonus miles from AT&T it is best to go to the ATT.com website and access the corresponding offer then contact the phone number provided. Otherwise your request may be further delayed by contacting the wrong person or department. It's best to contact the phone number provided on the website or mail piece you may have received. This way you won't get the run around and spend too much time on the phone speaking to representatives who are not able to help you with an issue they do not specialize in.

Except, in the case of the internet service, when there is no live person to talk to, and any live person you reach tells you you've reached the wrong person. I finally gave up, and never received the miles.
It's not worth the hassle--don't even try ATT.
