Called several numbers mentioned in the thread, finally got the right one, deal was valid, got the card, the first statement and the promised 10k miles showed up almost simultaneously. Only suggestion from my experience: Call during weekday working hours when the phones are not being manned by $5/hr untrained part-time employees.

With fewer than four scores and three dozen active brain cells left, I can't remember which phone no. finally got me to the right person, as I tried several. Sorry. And I didn't know about any codes, just told them that I wanted the business card for which they waive the first year's service charges and also give 10K miles. Most CSRs I talked to, hadn't ever heard of a crazy thing like that. But when it clicked and I got the right person, the guy signed me up in less then 2 minutes. Perseverance pays. Good luck.
