Update as of May 21, 2014 - in recent weeks some people have reported being told that they are no longer eligible for this offer, in spite of meeting the previous terms and conditions. Also, if you have high net worth, a "High Net Worth supervisor" may be able to help - simply call the Premium Services number on your account statement to find someone who has the power to do most anything. Get the name and phone number of the person you spoke with and ask him or her to note in your account that you have been approved, and by whom. Then, if after you transfer assets you get an email saying you are not eligible for the offer for which you registered (yes, this has happened), call the rep again, asking to read the notes on your account. You will probably be told that the email was automated and was sent in error and that they will hand babysit the funding and resultant miles.
If the registration page tells you are not eligible because you have had the offer before, call one of the above numbers. You are eligible after on year. That year probably starts counting when the miles are awarded, not when you register or when you add assets.
Trusts require extra caution on their part. Sometimes they require that you give them a lot of paperwork for trusts. If you have a local Fidelity office, just bring in everything required and they will copy and mail it for you. Easy. Otherwise, just mail it in. If upon trying to register online you are informed that your Revocable Living Trust account is not eligible, well, just call one of the above numbers, because it is eligible. They just have to verify a few things and then override the system. They want to know that you are the only trustee and the only beneficiary.

Has anyone else had success in getting an exception by calling? I just tried to register and was denied since I'm well within the 12 months (July 2016 was my last time). Would it be worth a call? I'd drop $100K in there right away and keep it there as I have with all my deposits over the past 3 years. Haven't removed a cent from Fidelity if that would make a difference.

ditto. I last did mine in October of last year. Called the number in the Wiki. The frontline rep said no dice, so I asked him to ask anyway as a courtesy exception. Lo-and-behold the exception was granted, I am registered and will xfer the monies in next week

Something else about this. If this is ending, what's the point of leaving your money in the Fido account now? Before, if you took the money out before the 9 month period, you were barred from doing this promotion in the future. But now, there is no future, so what's to stop everyone from withdrawing their money prior to 9 months since there is nothing to be barred from?

I am thinking about doing another deposit again with the exception....tho I would only be able to get enough for 25K, so might not be worth it.

Actually, on a pro rata basis, the $25K deposit yields the best rate of miles. You can earn 15K miles. I'm going to do one as this will be our last opportunity to earn miles in this easy way.

Maybe because we had little activity? One account was stocks/mutual funds. The other was cash instruments (CDs). We didn't do any active trading.
