Club Carlson is excellent - no fee, can transfer any random number (so can zero out an account completely) and you can transfer to anyone regardless of surname or address.
Remember that all airline schemes let you book for someone else with your miles. Schemes which also allow one way bookings are even more convenient for clearing out small balances as the minimum miles needed is lower.
