Medallion members receive unlimited, free upgrades on any domestic fare, and as a PM I get upgraded 95% of the time on el cheapo fares.

Audio and video (including free movie on longer flights) on most aircraft (NWA, for example, has no A/V on any domestic aircraft).
Unlimited, free booze in Crown Room Clubs.
