12 hours at ICN
Have a layover 6 am - 6 pm coming up this winter (late Dec). Wondering what we should do that isn't too adventurous but also not hanging around in the airport for the whole time. I was thinking of going into the city in the morning and then grabbing a cheap hotel somewhere on the way back so we can take a nap and shower in the afternoon before boarding the 10 hour flight home. We'll have a 2 year old in tow so have to keep our expectations in check for how much we'll be able to see. Any advice is appreciated.

You can easily head into the city by train or limousine bus. To avoid traffic, I'd recommend the train. With a two year old, it might be better to take an express train as you have a guaranteed seat. The all stop train is more frequent and only 10 minutes longer, but it is subway style so a seat isn't guaranteed.
You can take a taxi from Seoul Station to Gwanghwamun. There you can explore Gyeongbokgung Palace, Insadong, Hanok Village etc. See the main sights, try some Korean food, shop for anything. This is also the downtown area so it is a good city vibe as well.
You don't need to get a hotel for a shower. The airport has free shower rooms airside to use like you'd find in an airline lounge. They're on the 4th Floor Rest & Relax area. There's also a traditional Korean sauna in the basement landside.
BUT if you want to go to a sauna you should try Siloam conveniently located behind Seoul Station. They have excellent facilities, resting areas if you want a nap and even very good food! Around 10$ per person for entry.

All good suggestions from mikesaidyes. I might add a few more.
There are lots of affordable and good quality hotels in the city if you want a place to relax during your layover. I prefer to use hotels.com to check for decent places to stay. The reviews by other travelers will give you a good feel for the hotel. Myeongdong is an area you might consider because everything is very close by. There are many airport limo buses that stop at various hotels in the area. Taking the limo bus back to the airport is a good way to get in a short nap!

You might save money if you can get a hotel to give you a day rate, assuming that the times you will need the room work for a day rate.

Great info as always anyone. My partner and I will be doing a similar itinerary and were wondering if we have enough time for a quick DMZ tour? Are there are tour groups that would be willing to pick us up from the Airport for this? We arrive at 06:00 and depart at 20:00. Unfortunately this is on a Sunday so I'm not sure if that eliminates that possibility.

You're traveling with a 2-year-old, who will need naps and such.
I'd suggest staying at an inexpensive place not far from the airport; there are many. You would end up with much less moving about, taking of trains, and crowds.
Just did a Google search. One place that came up is near "Wolmido Amusement Park, and Songwol-dong Fairy Tale Village". Might be perfect for your young child. It's $39.

I've never stayed there, but have stayed at nice, inexpensive places in the Incheon (airport) area.

Any restaurant recommendations in the airport? Does the cinema play first run movies with English subtitles at all?
OP gets there at 6 AM. Is anything open other than shops? I get there at 4:30 AM so have a similar problem. The free showers don't open until 8 AM, right?

The CGV will play English movies in English with Korean subtitles. Korean movies won't have anything but Korean speaking.

The free showers are airside in the main terminal building on the 4th floor. If you want to exit the airport and visit the capsule hotel, go to Seoul, CGV movie theater or even the Korean spa in the basement of the airport.... you cannot use those free showers. You'd have to stay inside the airport as a transit passenger.
Try the Korean spa in the basement for a shower if you don't want to go into Seoul. If you do go into Seoul, I highly recommend Siloam Sauna for a shower and sauna experience. Even the food there is gooooood!

I have a similar itinerary, but departing 2pm. (Still arriving 6am).
If I were to venture into the city for a few hours, what is required from an immigration standpoint? Do I need a visa, or will my USA passport be sufficient?
Upon returning to ICN, how much time should I allow to get back thru security?
Thanks!

Not sure about the return time. I didn't have a very long wait at immigration or security when I departed several months ago, but that was early morning.

I would plan on being back at the airport 1:30 before the flight, assuming you
are already checked in and have a boarding pass.

Arriving at the airport at 1:30 for a 2 flight is totally impossible. If your flight is at 2, I would arrive around 12:45-1. If you dont have to check bags or do anything. If you have to check bags and get boarding pass, then 12:15-12:30.
US airlines board really early (like an hour) and KE/OZ just 30 mins before departure. That time of day isnt super busy time, but 1:30 leaves NO time for delays in security. Its also a 5-10 Min walk from the train terminal to the airport building itself.
If you have a US passport, there is no visa requirement. You get 90 says visa free entry.
