The agent from whom you purchase the Rail Pass will probably send you one with the exchange order. Even if they don't it only has 3 or 4 lines and can be filled out in about a minute.
I don't know if this is still current or is printable. The last time I took one with me they told me it had changed and I had the old one. I filled out a new one they gave me very quickly while the agent was looking at my passport.

The form posted above doesn't look anything like the one I had to fill out in NRT last month. Depending on where you exchange your voucher you are likely to spend more time standing in a queue than filling out the form. In NRT there is an usher who gives you the form to fill out while you wait for your turn. As abmj-jr confirmed it only takes a minute to complete. Just relax and enjoy your trip to Japan.

All they really care about is that you have a tourist stamp on your passport (temporary admittance, the length of which varies depending on your country of origin), not a working visa or other form of legal long-term residence.
The form itself is not complicated. If you want to avoid waiting, pay cash or credit card for your transport into Tokyo and validate your pass the next day at any station that has a Midori-no-Madoguchi (ticket office indicated by a green four-leaf clover sign).

I would like to activate as quickly as possible as my plan is to catch the first LEX to Tokyo Sta. then on to Kanazawa by Shinkansen. I need to activate, get assigned seats for both trains, get to Kanazawa Sta. to request assigned seats for the rest of my trip.

I can only speak about T1 but that time of the morning there shouldn't be a long queue and you will probably make the 1048 or even earlier N'EX and definitely 1118 or 1148. No need to worry about the form.

You will have plenty of time. The new shinkansen to Kanazawa only takes +/- 2-1/2 hours. You will be there by early to mid-afternoon.
